海尔曼螺杆菌感染的致病性研究

摘　　要：目的　调查海尔曼螺杆菌在人胃内感染的致病性和致病特点。方法　对 12 92例胃镜受检者进行Hh的形态学、生化检测及组织病理学检查 ,并观察Hh对常规抗Hp药物 (胶体 钅我 +甲硝唑 +四环素 )的反应。结果　 12 92名受检者检出Hh阳性者 2 2例 ,其中 17例有消化道症状 ,内镜下均为慢性胃炎表现 ;病理检查见所有Hh感染者均有散在的淋巴细胞 /浆细胞浸润 ,9例见中性粒细胞浸润 (9/ 2 2 ) ,6例见淋巴滤泡形成 (6 / 2 2 ) ,炎症程度总体上较Hp感染轻 ;Hh主要定植于胃窦 ;2 2例中尿素酶试验阳性者 15例 ,其中 11例弱阳性 ;14例Hh阳性者经抗Hp三联治疗 ,13例细菌清除 ,余 1例持续感染 ;另有 2例Hh阳性者未经任何治疗细菌自然消失。结论　Hh的致病性远弱于Hp ,尿素酶活性低 ,主要引起胃窦部的慢性浅表性胃炎 。Aim To investigate the infection characteristic of Helicobcter heilmannii(Hh).Method Endoscopic examination were exerted to 1292 individuals.Morphological methods such as Gram staining of biopsy smears,Giemsa staining or Warthin Starry silver staining of paraffin embedded slides and rapid urease test(RUT) were used to identify Hh.HE stained slides were observed for histopathological changes.The routine anti Hp regimens(CBS+MTZ+Tet) were used to treat Hh infection.Results Twenty two cases of Hh infection were found in 1292 people.17 out of 22 had the gastroenterologic symptoms and all 22 showed chronic gastritis under endoscopy.Sporadic lymphoplasmic infiltration was found in all patients with Hh infection.Formation of lymphoid follicles(6/22) and infiltration of neutrophils(9/22) could be found too.15/22 of the gastric tissues were RUT positive.Hh was eradicated in 13 out of 14 with the triple therapeutic regimens.And in two patients,Hhdisappeared spontaneously without any treatment.Conclusion Hh has lower urease activity and weaker pathogenicity when compared with Hp.It mainly causes chronic antrum gastritis.The infection of Hh is easy to eradicate through drug therapy and even cures spontaneously.
