直肠癌根治术采用075%罗哌卡因和05%布比卡因行硬膜外阻滞的比较

摘　　要：目的　比较 0 75 %罗哌卡因和 0 5 %布比卡因对直肠癌根治术硬膜外麻醉的临床效果 ,观察为罗哌卡因临床应用提供客观依据。方法　 30例直肠癌 (ASAⅠ～Ⅱ级 )随机分二组 :Ⅰ组 0 75 %罗哌卡因 (n =15例 ) ;Ⅱ组 0 5 %布比卡因 (n =15例 )。取L2～ 3行硬膜外穿刺 ,向头端置管 4～ 5cm ,注入首次剂量罗哌卡因或布比卡因 8～ 15ml,总量不超过 2 0ml。结果　两组病人用药总量 ,感觉阻滞范围和总有效时间无明显差异。Ⅰ组较Ⅱ组止痛和肌松效果好 ,对血压和心率影响小。结论　与 0 5 %布比卡因比较 ,0 75 %罗哌卡因感觉阻滞完善 ,止痛和肌松效果良好 ,对循环功能影响小 ,不良反发生率低。Objective To investigate the effects of 0.75% Ropivacaine compared with 0.5% bupivacaine for epidural anesthesia in rectal cancer section in order to provide objective basis for clinical application of Ropivacaine.Methods 30 patients (ASAⅠ～Ⅱ)were divided into two groups randomly:Group Ⅰ:0.75% Ropivacaine (n=15),group Ⅱ:0.5% Bupivacaine (n=15).In all the patients epidural space L 2～3 was selected,then the catheter was put upward 4～5 cm.In five minutes the initial dose of Ropivacaine/bupivacaine 8～15 ml was injected.The total dose of study drugs was less than 20 ml.Results There was no significant difference in total dose,ramge and operational time of sensory block between the two groups.But in group Ⅰ more satisfactory analgesia and muscle relaxation were obtained with less change of blood pressure and heart rate.Conclusions Compared with 0.5% Bupivacaine,0.75% Ropivacaine provided satisfactory sensory block,analgesia and muscle relaxation with less effect on cardiovascular function and fewer adverse events.
