脊髓髓内肿瘤并脊髓空洞症的外科治疗及临床评价

摘　　要：目的　探讨脊髓髓内肿瘤并脊髓空洞症的外科治疗及临床评价。方法　对 1 3例手术治疗患者的临床资料进行回顾性分析。结果　合并脊髓空洞的髓内肿瘤的手术全切率为 76 92 % (1 0 / 1 3 ) ,术后空洞缩小或消失率 61 54%(8/ 1 3 ) ;术后感觉异常及疼痛改善不明显 ,而以肌力及大小便功能恢复为主 ,临床症状轻重与脊髓空洞大小无关 ;除星形细胞瘤外其它肿瘤术后增生恶化或复发率均较低。结论　脊髓空洞的存在有利于髓内肿瘤的切除 ,且术后空洞常可自行恢复正常 ,早期诊断、手术全切是治疗成功的关键。Objective To study surgical treatment and clinical evaluation of intramedullary tumors with accompanying syringomyelia.Methods Clinical data of 13 cases were analyzed restrospectively.Results The rate of total tumor resection in patients with accompanying syringomyelia was 76.92%(10/13).The rate of diminution or disappear ance of syringomyelia was 61.54%(8/13).Postoperative pain and dysesthesia were generally unchanged,but motor weakness,bladder andbowel function were improved.The severity of symptoms was not related to the of syringomyelia size.Except for astrocytomas,the rates of recurrence and aggravation of othertumors were low.Conclusions The presence of syringomyelia favours resectability of intramdeullary tumors,the accompanying syringomyelia disappears automatically when the tumors have been resecfed.It is very important to make early diagnosis and total resectcon of intramednllary fwmors.
