保留髂淋巴管预防子宫颈癌根治术后淋巴囊肿的临床观察

摘　　要：目的　观察保留髂淋巴管预防子宫颈癌根治术后淋巴囊肿的临床效果。方法　对 86例ⅠaⅡa期子宫颈癌患者在行根治性子宫切除时 ,保留髂淋巴管主干 ,术后用B型超声等手段观察淋巴囊肿的发生情况 ,并观察对远期存活率的影响。结果　 86例中无一例发生盆腔淋巴囊肿 ,2例在腹股沟区出现直径约 2cm大小的淋巴囊肿 ,发生率为 2 3% ,5年存活率为 93 5 %。结论　在行子宫颈癌根治术时保留髂淋巴管主干 ,可有效地防止盆腔淋巴囊肿的发生 ,且不影响远期存活率。Objective To clinically evaluate the value of the remaining iliac lymphatic vessel to prevent lymphatic cyst following radical resection for cervical cancer.Methods Iliac lymphatic trunks were remained during the radical resection in 86 patients of cervical cancer with stage Ⅰa~Ⅱa.The incidence of lymphatic cyst and the effect on the long term survival rate were observed.Results None of the 86 cases suffered from pelvic lymphatic cyst.2 cases suffered from inguinal lymphatic cyst which was about 2cm and the incidence rate was 2 3%.The 5-year survival rate was 93 5%.Conclusion Remaining iliac lymphatic trunk in radical resection for cervical cancer can prevent the incidence of pelvic lymphatic cyst effectively without affecting long term survival rate.
