内眼手术对糖尿病患者血糖的影响

摘　　要：目的评价糖尿病患者内眼手术后血糖变化。方法选择内眼手术糖尿病患者３９例（４２眼），测定其术前、术后１～２ｈ，１，３，５天空腹血糖。结果糖尿病患者内眼手术后血糖显著高于术前，术后１～２ｈ达到高峰，后逐渐下降（Ｐ＜０．０５），至５天基本接近术前水平（Ｐ＞０．０５）。血糖升高与手术时间、术前血糖水平、糖尿病病程、麻醉满意程度呈正相关（ｒ＞０）。结论糖尿病患者内眼手术后血糖变化存在着先升高，至１周左右趋于术前水平的规律。ObjectiveTo evaluate the change of blood sugar in diabetic patients after intraocular surgeries,and to analyze the relative factors.Methods39 patients(42 eyes)who underwent the intraocular surgeries received the tests of blood sugar preoperatively,and at 1～2h,1,3,and 5d postoperatively.ResultsPostoperative blood sugar increased significantly compared with the preoperative one(P<0.05),and reached the peak at 1～2h postoperatively,and then declined.There was no significant difference between preoperative blood sugar and that at 5dpostoperatively(P>0.05).There was a positive correlation between blood sugar increase and surgery time,preoperative blood sugar level,diabetic duration,and satisfactory anesthesia(r>0).Level of blood sugar is markedly influenced by the diabetic complications and patients’ emotion.The complications included anterior chamber fibrous exudates(9 eyes),hyphema(4 eyes),vitreous hemorrhage(4 eyes),deteriorative diabetic retinopathy(3 eyes),and suprachoroidal hemorrhage(1 eyes).ConclusionThere is regularity that the blood sugar increases just postoperatively and reaches the preoperative level approximately 1 week postoperatively.Adequate preoperative estimate of diabetic complications, satisfactory anesthesia,shorter surgery time,and good control of postoperative complications should be obtained.
