幽门螺旋菌与慢性胃炎、消化性溃疡及胃癌的关系

摘　　要：本文对325例因上消化道症状行内镜检查的患者的胃粘膜组织进行幽门螺旋菌(HP)捡查。其 HP 总检出率为70.2%,尿素酶试验、直接涂片、改良 Warthin—starry染色、细菌培养检出率分别为69.9%、52%、75.1%63.1%HP 在 CSG、CAG、Gu、Du、GCa 及 N 中的活检率分别为70.1%、70.6、63%、87%、23.1%7.1%。结果表明以改良W—S 银染检出率最高,直接涂片检出率最低,直接涂片法与其它三种方法比较差异有非常显著性(P<0.005)。HP 在 CSG、CAG、Gu、Du 中检出率较 GCa、N 组高,差异有非常显著性(P<0.005);GCa 组与 N 组比较,差异无显著性(P>0.005)。说明 HP 与慢性胃炎、消化性溃疡有密切关系,与胃癌关系不大。Departmont of Microbiology,Hengyang Medical College In this paper,the detection of Helicobaeter pylori infection was prformed on gastric mucosa obstained from biopsises of 325 patients undergoing endoscopy for various upper gastrointestinal diseases.Helicobacter pylori infection was detected by methods including direct smear,modified warthin-Starry stain,urease test and bacteria culture.The positive values for the above methods were 50.2%,75.1%,69.9%,63.1% respectivoly.The general incidence of Helicobacter pylori infection on 325 patients was 70.2%.The incidence of Helicobacter pylori infection in chronic superficial gastritis(CSG),chronic atrophic gastrinis (CAG),gastric ulcer(GU),duodenal ulcer(DU),gastric carcinoma(GCa),and normol(N)group were 70.1%,70.6,63.0%, 87.0%,23.1%,and 7.1%,respectively.The results showed that the modified Warthin-Starry Stain was the most sensitive among the above methods,while the direct smear showed the lowest positive value.Our finding also confirmed that the prevalences of Helicoba- cter pylori in CSG,CAG,GU,and DG group were higher than that in GCa and N group(p<0.05),but the prevalence of Helicob- acter pylori in the GCa group was similar to that in the N group (p>0.05).In conclusion,Helicobacter pylori infection is closely associated with chronic gastrithis and peptic ulcer,but shows no relation with gastric carcinoma
