血清CA125检测在肺癌中的价值

摘　　要：目的 :了解血清 CA12 5水平在原发性肺癌患者预后判断中的价值。方法 :对 5 0例健康人、6 4例肺部良性疾病患者、130例未经治疗的肺癌患者进行血清 CA12 5测定。结果 :CA12 5平均浓度在肺癌患者中为 (76 .9± 118.37)× 10 3U/ L。在肺良性病变中为 (2 2 .5 2± 2 2 .5 5 )× 10 3U/ L,两者间差异非常显著 (P<0 .0 1)。在健康人群中 CA12 5平均浓度为 (2 .6 5± 3.2 9)× 10 3U/ L。以 6 0× 10 3U/ L 为临界值 ,肺癌组与正常组比较 :敏感性为 34.6 %、特异性为 10 0 %、阳性预测值 10 0 %、阴性预测值 37%、正确性 5 2 .8%。肺癌组与肺良性病变组比较上述值分别为34.6 %、92 .19%、90 %、40 .97%、5 3.6 %。本组资料中几乎所有 CA12 5水平升高肺癌患者都是晚期 (4 3/ 45 ) ,其生存率明显低于 CA12 5水平正常的肺癌患者 ,两者间差异非常显著 (P<0 .0 1)。结论 :血清 CA12 5是一个显示疾病程度的指标 。Objective:To study the evaluation of serum CA125 as a tumor marker in lung cancer.Methods:Serum CA125 levels were evaluated in 50 healthy subjects,64 patients with benign pulmonary conditions and 130 patients with untreated lung cancer.Rusults:The mean concentration of CA125 was higher in patients with lung cancer[(76.9±118.3)×10 3U/L] than in those with nonmalignant disease[(22.52±22.55)×10 3U/L] ( P <0.01) .In the healthy control group,CA125 concentrations were significanctly low(2.65±3.29)×10 3U/L]. At a cut off value of 60×10 3 U/L,CA125had a sensitivity of 34.6%,specificity of 100,positive predictive value of 100% and negative predictive value of 37%,correctness of 52.8% with respect to healthy subjects.In patients with benign pulmonary conditions,those values were 34.6%,92.19%,90%,40.97% and 53.6% respectively. Conclusion:CA125 is a good indicator of disease extent and serum level correlated to the length of survival.
