摘 要:为探讨上皮钙粘附素 (E -cad)和β连接素 (β-cat)表达与膀胱移行细胞癌分化、侵袭力和预后的关系 ,应用免疫组化法检测 6 0例膀胱移行细胞癌标本中的E -cad、β-cat表达情况。结果表明 :E -cad及 β -cat在G1、G2 和G3 级肿瘤的正常表达率分别为 73.3% (18/ 2 3)、5 0 .0 % (9/ 18)、2 6 .3% (5 / 19)和 82 .6 % (19/ 2 3)、44 .4% (8/ 18)、2 1.1% (4/ 19) ,在Tis-T1、T2 ~T4 期肿瘤中的正常表达率分别为 78.3% (18/ 2 3)、37.8% (14/ 37)及 82 .6 % (19/ 2 3)、32 .4% (12 / 37) ,E -cad、β-cat表达与病理分级、临床分期比较差异有高度显著性 (P均 <0 .0 1) ,E -cad在正常表达组、异常表达组的平均生存期分别为 95月、6 3月 ,β-cat正常表达组、异常表达组的平均生存期分别为 10 0月和 71月 ,生存期两者差异有显著性 (P <0 .0 1或P <0 .0 5 )。认为E -cad和β-cat异常表达与膀胱移行细胞癌恶性进展过程有关 ,二者均可作为判断肿瘤恶性程度及预后的指标。In order to investigate the possible relationship between E-cadherin (E-cad) and β-catenin (β-cat) expression and the differentiation, invasiveness and prognosis in bladder transitional cell carcinoma (BTCC), an immunohistochemical method was used to examine the expression of E-cad and β-cat in 60 paraffin-embedded urinary bladder transitional cell carcinoma specimens. The normal expression rates of E-cad and β-cat in pathologic Grade 1, Grade 2 and Grade 3 were 73.3% (18/23), 50.0% (9/18), 26.3% (5/19) and 82.6% (19/23), 44.4%(8/18), 21.1%(4/19), respectively. The normal expression rates in Tis~T 1 and T 2~T 4 were 78.3%(18/23), 37.8%(14/37) and 82.6%(19/23), 32.4%(12/37), respectively. In comparison between the above data, the differences were statistically significant ( all the P values were <0.01). The mean survival periods of E-cad in the normal group and the abnormal group were 95 months and 63 months, respectively, and the mean survival period of β-cat in the normal and the abnormal group were 100 months and 71 months, their differences being statistically significant (P<0.01 and 0.05, respectively). This study indicates that aberrant expression of E-cadherin and β-catenin is related to the malignant progression of BTCC, and can be used as markers of malignant potential and prognosis of BTCC.
