rhTNF-α联合IL-2体外净化急性髓细胞性白血病骨髓的实验研究

摘　　要：目的 :探讨重组人肿瘤坏死因子 α(rh TNF- α)联合白细胞介素 2 (IL- 2 )体外净化 AML 骨髓方案的可行性及其作用机制。方法　联合 rh TNF- α(1× 10 6 U/ L)和 IL- 2 (5× 10 5 U/ L) ,对 17例 AML 患者骨髓体外净培养1天～ 3天 ,采用半固体培养法测定粒—巨噬细胞集落 (CFU- GM)、白血病祖细胞集落 (CFU- L)形成 ,MTT细胞毒性测定及免疫组化法测定 bcl- 2阳性率的变化。结果　 1rh TNF- α与 IL- 2联合后 ,对 CFU- L 有选择性抑制作用 ,而对 CFU- GM无明显影响 ;2 rh TNF- α和 IL- 2对白血病细胞有协同杀伤作用 ,净化培养 3天后 ,bcl- 2阳性率明显降低。结论　联合 rh TNF- α和 IL- 2净化效果好 ;介导细胞毒性 ,下调 bcl- 2表达 ,诱导白血病细胞凋亡 ,可能是其净化骨髓的机制之一。Objective:To explore the possibiliy and mechanism of purging myelogenous leukemia marrow in vitro with rhTNF-α and IL-2. Methods: We selected randomly 17 patients with acute myelogenous leukemia (AML) to purge bone marrow in vitro with combination of rhTNF-α (1×105U/L) and IL-2 (5×105U/L). Granulocyte-Macrophage colony-form unit(CFU-GM), Leukemia colony-form unit(CFU-L) was examined by semisoid culture method. Cytotoxicity assay was examined by MTT. The expression of bcl-2 was studied by immunohistochemistry. Results: ①rhTNF-α in combination with IL-2 may have selective killing effect on CFU-L without influencing on CFU-GM; ② The selective effects of inhibiting AML cells was obvious with the combination of the two cytokines, the expression of bcl-2 decreased for 3 days' coculture.Conclusion: The prelimitary study suggests that rhTNF-α and IL-2 had better purging effect on AML cells;inducing cytotoxicity, down regulating the expression of bcl-2 and inducing leukemia cells to apoptosis may be one of the mechanisms of rhTNF-α and IL-2 purging bone marrow in vitro.
