两类缺血性视神经病变眼底荧光血管造影早期视盘充盈探讨

摘　　要：目的 观察比较非动脉炎性前部缺血性视神经病变(NAION)与前部缺血性视神经病变(AION)的眼底荧光血管造影(FFA)早期视盘充盈的显著性差异及特殊性,以提示临床上对这两类病变的早期鉴别诊断。方法 用FFA常规技术,随机抽取正常对照组28例,视盘血管炎12例,急性视神经炎12例,前部缺血性视神经病变27例进行统计学对照分析。结果NAION与正常对照组早期平均荧光充盈时间无显著性差异(P<0.10-0.20),AION与正常对照组早期平均荧光充盈时间延长2秒钟以上(P<0.01-0.05),视网膜中央动脉早期显影时间至少延长3秒钟以上,具高度显著性差异(P<0.001)。结论 仔细观察视盘筛板前区FFA早期血管充盈模式,提供早期鉴别NAION与AION两类病变的诊断依据。Objective To observe significant difference in the early filling patterns of optic disc in fundus fluorescein angiography (FFA) between nonarteritic anterior ischemec optic neuropathy(NAION) and arteritic ischemic optic neuropathy (AION). Methods By routine FFA technique, 28 normal controls, 12 cases of optic disc disc angeitis, 12 cases of acute optic neuritis and 27 cases of anterior ischemic optic neuropathy were selected and a statistic contrastive analysis was made. Results Compared with normal controls, NAION group did not show significant difference in the onset and completion of filling (P<0.10 - 0.20); AION group revealed significant delay in the onset and filling of the prelaminar disc (P<0.01 -0.05) as well as significant delay in the development of central retinal artery by at least 3 seconds (P<0.001). Conclusion Careful examination of the early angiographic filling patterns of the prelaminar optic disc may aid in the differentiation between NAION and AION.
