准分子激光屈光性角膜切削术治疗近视、散光术后3年疗效分析

摘　　要：目的 为了评价准分子激光屈光性角膜切削术治疗近视、近视散光的远期疗效。方法 应用美国VISX 20/20型准分子激光仪,治疗132例(259眼),采用多切削区治疗屈光度-1.5D～-16.0D的近视观察3年以上,按术前球镜屈光度分为二组:Ⅰ组-1.5D～-6.0D(196眼),Ⅱ组-6.25D～-16.0D(73眼)。结果 术后3年裸眼视力≥0.5,在Ⅰ、Ⅱ中组中分别为94.6％、89.3％;术后3年裸眼视力≥1.0,在Ⅰ、Ⅱ组中分别为90.7％、70.5％,角膜HazeⅠ组0级90.3％,0.5级9.7％,无1级以上Haze;Ⅱ组0级63.0％,0.5级34.2％,1级2.8％,无2级以上Haze。结论 准分子激光屈光性角膜切削术对低、中、高度近视、近视散光均取得良好的远期效果,但对低、中度近视效果更佳,对高度近视预测性较差。Obiective To evaluate the long - term effect of the excimer laser photorefractive keratectomy for myopia and astigmatism. Methods 93 cases(186eyes)of- 1.5D~ -6.0Dmyopia and 39 cases(73eyes)of- 6.25D~ - 16.0D myopia were treated with a excimer laser apparatus (VISX 20/20type U.S.A) .The follow- up period was 3 years.Results The rate of post- operative vision above 0.5 is 94.6% in the first group and 89.3% in the second group.The rate of post - operative vision above 1.0 is 90.7% in the first group and 70.5% in the second group.The rate of cornea Haze in the first group:90.3% in degree 0, 9.7% in degree 0.5 and none in degree 1or2. The rate of cornea Haze in the second group: 63% in degree 0,34.2% in degree 0.5,2.8% in degree 1 and none in degree 2. Conclusions Excimer laser photorefractive keratectorny for all myopia and astigmatism can achieve long- term satisfactory effects.The effects on low or middle myopia is better while the prospective evaluation on high myopia is worse.
