老年人食管癌、贲门癌的外科治疗

摘　　要：目的 :总结老年人食管癌、贲门癌的外科治疗经验。方法 :2 6 4例中手术切除肿瘤 2 47例 ,切除率为 93.5 6 %。其中胃食管弓上吻合术 12 6例 ,弓下吻合术 93例 ,右侧胸顶部吻合术 10例 ,颈部吻合术 18例。结果 :吻合口瘘 2例 ,其中 1例死于呼吸衰竭 ,1例治愈。肺部感染 2 1例 ,且有 1例死于呼吸衰竭。心脏并发症 14例。结论 :通过加强围术期的管理 ,老年人食管癌、贲门癌的外科治疗效果是满意的。Objective:To summarize the experience in surgical treatment of carcinoma of esophagus and cardia in the elderly.Methods:The tumours in 247 patients were resected(93.56%).The anastomosis of gastroesophagus was performed at above the arch of aorta in 126 cases,at under the arch of aorta in 93 cases,at the top of right thoracic cavity in 10 cases and at the neck in 18 cases.Results:Two patients manifested leaks of anastomoses.One died of respiratory failure and the other was cured.The complications of lungs were found in 21 cases and one of them died of respiratory failure.The complications of heart were observed in 14 cases.Conclusions:With careful perioperative management,surgical treatment of carcinoma of the esophagus and cardia in the elderly may achieve a satisfactory result.
