动脉灌注磷-32玻璃微球治疗口腔癌的初步应用评价

摘　　要：首次采用作者联合研制的磷-32玻璃微球(P-32GMS)经颈外动脉分支灌注治疗口腔鳞状细胞癌共10例,其中舌癌9例,颊癌1例,均为 T_3～T_4。P-32GMS 的剂量为每例6000～10000rad。舌癌者经舌动脉,颊癌者经面动脉插管,染色定位后,行 P-32GMS 一次性灌注。结果:8例癌瘤完全消失,2例癌瘤缩小50%以上。无一例出现白细胞降低,骨髓抑制和肝、肾、肺、或大脑功能障碍以及其它毒、副作用。P-32GMS 颈外动脉分支灌注治疗口腔癌具有疗程短,效果好,方法简易、安全、可靠等优点,是一种有发展前景,有效而合理的抗癌疗法。A 46～76 μm P-32 GMS has been developed in China.The intraarterial infusion of P-32 GMS is a new approach to the treatment of malignancy.The present paper is the firstreport of clinical application of P-32 GMS to oral cancer based on a serial experimentalstudy.P-32 GMS were injected via a certain branch of external carotid artery with acatheter on 10 patients with advanced squamous cell carcinoma of oral cavity(tongue 9,baccal area 1,stage Ⅲ & Ⅳ).60～100 GY(6000～10000 rad)doses of P-32 GMS wasgiven for per case and the response was evaluated in each of them.The following-uprange was 3～16 months Encouraging result was obtained.8 of them were regarded asexcellent(Complete response),2 were partial response(greater than 50%).No toxicityand complication,especially no microspheres distributed to the bone marrow and nomyelosuppression were encountered in this group.We may conclude that:P-32 GMSadministration preceded by regional infusion can potentially yield a synergistic,highlyselective attack on the target tissue and tumor.The large absorbed doses of P-32 GMS(6000～10000 rad)can be tolerated by the target organ,but strong enough to control themalignancy.Such infusion seems safe and simple.This internal radiotherapy may becomea feasible and effective approach to the treatment of oral cancer and other malignancy.
