摘 要:目的 比较99Tcm tetrofosmin和99Tcm 甲氧基异丁基异腈 (MIBI)显像诊断乳腺癌和腋淋巴结转移的临床价值。方法 对 5 2例乳腺肿瘤患者进行99Tcm tetrofosmin和99Tcm MIBI显像。患者均在 2周内行外科手术治疗 ,并做病理检查。结果 2 6例患者病理检查证实为乳腺癌 ,其中 30例为良性病变。99Tcm tetrofosmin显像发现乳腺癌 2 1例 ,其中 11例合并腋淋巴结转移 ;99Tcm MIBI显像发现乳腺癌 19例 ,其中 12例合并腋淋巴结转移。99Tcm tetrofosmin和99Tcm MIBI显像对乳腺癌诊断的灵敏度、特异性和准确性分别为 80 8%、76 7%、78 6 %和 73 1%、73 3%、73 2 % ;诊断腋淋巴结转移的灵敏度、特异性和准确性分别为 6 8 8%、80 0 %、76 0 %和 75 0 %、80 0 %、76 9%。结论 99Tcm tetrofosmin显像对乳腺癌的检出优于99TcmObjective To evaluate the clinical diagnostic value of 99Tcm-tetrofosmin in the imaging of breast carcinoma and axillary lymph node metastases and to compare with that of 99Tcm-MIBI Methods 52 females with breast masses were examined by 99Tcm-tetrofosmin scintigraphy and 99Tcm-MIBI scintigraphyAll the results were compared with that from pathologic study Results 26 patients were found to have a primary malignancy of the breast ,whereas 30 had benign disease 99Tcm-tetrofosmin breast imaging showed abnormal uptake in 21 of 26 malignancies and in 11 of 16 axillary lymph node metastasesThe sensitivity ,specificity, and accuracy values obtained with 99Tcm-tetrofosmin and 99Tcm-MIBI scintigraphy for breast carcinoma were 808%, 767%, 786%,and 731%,733%,732%, respectively The values obtained with 99Tcm-tetrofosmin and 99Tcm-MIBI scintigraphy for axillary lymph node metastases were 688%, 800%, 760%,and 750%,800%,769%, respectively Conclusions Both of the two techniques are effective in the differentiation of malignant breast mass-es from benign ones and in detecting axillary lymph node metastasesHowever , 99Tcm-tetrofosmin is superior to 99Tcm-MIBI in detecting breast carcinoma
