胃左动脉灌注化疗与手术综合治疗中晚期贲门癌

摘　　要：目的　对比研究单纯手术、单纯动脉灌注化疗和综合治疗对中晚期贲门癌的治疗效果。方法　 79例中晚期贲门癌患者 ,分成单纯手术 (A组 )、单纯动脉灌注化疗 (B组 )和综合治疗组 (C组 ) ,进行对比研究。其中A组 32例 ,B组 18例 ,C组 (先动脉灌注化疗后手术 ) 2 9例 ,3组平均年龄分别为 5 9.2岁、5 9.6岁、5 8.9岁 ,临床分期均为ⅢB期。结果　动脉灌注化疗 (B组和C组 )47例 ,半个月后 95 .7%患者症状改善 ,肿瘤缩小 ;肿瘤切除率 :A组为 6 2 .5 % (2 0 /32 )、C组为 10 0 .0 % (2 9/2 9) ;A组 1、3、5年生存率分别为 78.1% (2 5 /32 )、46 .9% (15 /32 )和 2 5 .0 % (8/32 ) ,C组分别为 93 .1% (2 7/2 9)、79.3 % (2 3/2 9)和 5 1.7% (15 /2 9) ,2组术后3、5年生存率比较有显著性差异 (P <0 .0 5 ) ;B组和C组 1年生存率比较 ,有非常显著性差异 (P <0 .0 1)。结论　综合治疗组的手术切除率和术后生存率显著高于同期的单纯手术组和单纯动脉灌注化疗组。动脉灌注化疗Objective To study the curative effect of surgery、left gastric arterial chemoinfusion and combined treatment for mid advanced cardial carcinoma.Methods In this study,79 cases of cardial carcinoma from January 1991 to November 1996 were reported,among them,32 cases were treated by surgery(A group),18 cases by left gastric arterial chemoinfusion(B group) and 29 cases by combined treatment(C group).The average age of A,B and C group was 59.2,59.6,and 58.9 years old respectively.All cases belonged to ⅢB stage.Results The symptoms of 45 cases(95 percents of B and C group) were improved after the treatment of arterial chemoinfusion and the volume of tumours was decreased.The tumours resectable rate of A,C group was 62.5%(20/32),100.0%(29/29),respectively.The 1,3,5 year survival rates in A group was 78.1%(25/32),46.9%(15/32) and 25.0%(8/32),respectively,in C group it was 93.1%(27/29),79.3%(23/29) and 51.7%(15/29),respectively.There was significant difference between A group and C group in 3,5 year survival rates( P <0.05),and between B group and C group in 1 year survival rate( P < 0.01 ).Conclusion The tumor resectable rate and the postoperative survival rate of C group are much better compared with group A and B.The suitable time of operation is 1 month after arterial chemoinfusion.
