腰动脉加髂内动脉栓塞在髂腰部肿瘤中的应用

摘　　要：目的 :探讨腰动脉加髂内动脉栓塞在髂腰部肿瘤治疗中的价值。方法 :采用选择性动脉插管 ,用经高温处理的明胶海绵栓塞供应肿瘤的腰动脉、髂内动脉 ,姑息性治疗的肿瘤患者同时给予动脉灌注。结果 :本组 2 3例中 11例栓塞后行手术治疗 ,术中出血量明显减少 ,平均为 3 60ml,且手术进行顺利 ,肿瘤切除彻底 ;12例不能手术的病例行灌注和栓塞术后 ,症状明显减轻或缓解 ,生活质量提高 ,延长了生存期。结论 :在髂腰部肿瘤的治疗中 ,选择性腰动脉加髂内动脉栓塞是一种安全有效的治疗方法。Objective:To evaluate the efficacy of iliolumbar tumors treated by both waist artery and internal iliac artery embolization.Methods:An arterial catheter was selectively inserted into waist artery and internal iliac artery.The insertion gelfoam fragments treated with hyperthermia were introduced through catheter to oculude the blood supply of tumors.Results:11 of 23 patients showed a reduction in intraoperative blood loss after embolization.(The average of intraoperative blood loss was only 360ml ).And all these operations were successful.12 non-operative patients also decreased pain of later tumor and improved living quality.Conclusion:It's an effective therapy in treating iliolumbar tumors with both selective waist artery and internal iliac artery embolization.
