外耳道囊性腺样癌12例临床分析

摘　　要：目的 :总结手术治疗外耳道囊性腺样癌的疗效。方法 :回顾分析 1 978至 1 995年收治的外耳道囊性腺癌 1 2例的临床资料。结果 :4例术前病理诊断为耵聍腺瘤 ,行单纯肿瘤切除术 ,术后 2～ 8年复发 ,分别行 2～ 4次手术 (乳突根治、下颌骨切除、颞骨次全切除术 ) ,术后病理诊断为囊性腺样癌 ,术后随访 2～ 8年 ,1例死亡。 6例术前诊断为囊性腺样癌 ,其中 2例行乳突根治术 ,1例行乳突根治加腮腺切除术 ,3例行颞骨次全切术 ,随访 2～ 1 0年 ,2例死亡。 2例术前未行病理检查 ,行单纯肿瘤切除术 ,2～ 3年后复发再行乳突根治术 ,术后病理诊断为囊性腺样瘤 ,随访 5年无复发。结论 :早期就诊和彻底手术是治疗外耳道囊性腺样癌延长患者生命的关键。Objective:To evaluate the effect of surgery for meatal adenoidcystic epithelioma.Method:The clinical data of 12 cases were retrospectively analysed.Results:4 cases were pathologically diagnosed as ceruminoma and 6 were adenoidcystic epithelioma before surgery.2 cases were not performed pathology examination.In this study,2 cases were performed simple tumorectomy but relapsed after 2 to 3 years,then they received radical mastoidectomy and were pathologically diagnosed as adenoidcystic epithelioma.After 5 years follow-up, they did not relapse.4 cases were pathologically diagnosed as ceruminoma before simple tumorectomy.After 2 to 8 years,they relapsed and were performed surgery including radical mastoidectomy,resection of jawbone,subtotal resection of temporal bone.They were pathologically diagnosed as adenoidcystic epithelioma after surgery and 3 cases of them were in good health during 2 to 8 years follow-up.The other 6 caese were pathologically diagnosed as adenoidcystic epithelioma before surgery.2 cases were performed radical mastoidectomy,1 case was performed radical mastoidectomy and parotidectomy.3 cases received subtotal resection of temporal bone.After surgery,3 cases were in good health during 2 to 7 years follow-up.Conclusion:Earlier diagnosis and complete operation are the keys to enlong life of the patients.
