摘  要:目的 :研究Chiari畸形 (CM ) /脊髓空洞症 (SM )患者小脑扁桃体下疝的程度与空洞大小之间的关系 ,探讨其发病规律。方法 :回顾性分析 16 2例CM/SM的MRI资料 ,测量小脑扁桃体下疝的长度 (L)、空洞的长度 (空洞沿椎体的节段数 )和宽度、对所有的计量资料进行统计学处理。结果 :L <5mm ,占 13.5 8% ,L =5~ 15mm ,占 75 .31% ,L >15mm ,占 11.11%。小脑扁桃体下疝的程度与空洞长度的相关系数r1=- 0 .0 96 1,P =0 .35 17,与宽度的相关系数r2 =- 0 .0 0 5 5 ,P =0 .95 78。结论 :小脑扁桃体下疝长度在 5~ 15mm之间者易伴发脊髓空洞 ,小脑扁桃体下疝的长度与空洞的大小无显著的线性相关。Objective: To study the relationship between the extent of cerebellar tonsil herniation and the size of syrinx in patients with syringomyelia associated with Chiari malformation, and to explore the regular rule of its occurrence. Methods: Retrospective analyses were made on the MRI data of CM/SM in 162 cases. The length of cerebellar tonsil herniation and the length and width of cavity were measured. Then the measurement data obtained were analyzed statistically. Results: The patients with the length of cerebellar herniation below the lower border of the foramen magnum shorter than 5 mm accounted for 13.58%; those patients with the length of cerebellar herniation equal to 5~15 mm constituted 75.31%; and the patients with the length of cerebellar herniation longer than 15 mm accounted for 11.11%. The correlation coefficient between the extent of cerebellar tosil herniation and the length of cavity was rl=-0.0961, P=0.3517 and the coefficient for the width of the cavity was r2=-0.0055, P= 0.9578 . Conclusion: Syringomyelia was found more frequently in patients with a herniation of 5~15 mm than in those with a smaller or larger herniation . There was no significant linear correlation between the extent of cerebellar tonsil herniation and size of syrinx.
