联合支气管动脉灌注和静脉路化疗治疗肺癌

摘　　要：目的 　观察联合支气管动脉灌注和静脉路应用CAP、CE交替化疗治疗肺癌的疗效及副反应。方法　76例肺癌 ,均经细胞学或组织学证实。其中非小细胞型肺癌 6 0例 ,小细胞型肺癌 16例。非小细胞型肺癌化疗以CAP方案为首 ,交替CE方案。小细胞肺癌化疗以CE方案为首 ,交替CAP方案。支气管动脉灌注 :采用Seldinger氏法 ,经皮股动脉穿刺插管 ,选择供瘤支气管动脉后缓慢灌注化疗药。静脉路化疗 :采用CAP方案时 ,静脉滴注CTX 6 0 0mg/m2 ,第 1天和第 8天。采用CE方案时 ,静脉滴注VP - 16 10 0mg ,第 1天到第 4天。结果　 非小细胞肺癌 ,完全缓解 (CR) 4例 ,部分缓解 (PR) 2 9例 ,总有效率为 (CR +PR) 55% ;小细胞肺癌 ,CR5例 ,PR7例 ,总有效率为 75%。结论　 联合支气管动脉灌注和静脉路应用CAP、CE方案交替化疗治疗肺癌 ,能提高疗效、减轻副反应。Objective To observe the curative effects and side reactions of combining bronchial artery perfusion with chemotherapy through alternating intravenous application of CAP and CE programs in the treatment of lung cancers.Methods 76 cases of lung cancers confirmed by cytology and histology consisted of 60 cases of non-parvicellular lung cancers and 16 cancers of parvicellular lung cancers. CAP program was first applied to the treatment of non-parvicellular lung cancers and was then alternated by CE program,while CE program was first applied to the treatment of parvicellular lung cancers and was then alternated by CAP program.According to the Seldinger method, Chemotherapeutic drugs were slowly perfused into the selected tumor-feeding bronchial artery after the intubation through percutaneous femoral artery. 600mg/m 2 CTX was dripped intravenously on the first day and the 8th day in the CAP program,while 100mg VP-16 was dripped from the first day to the fourth day in the CE program.Results In the non-parvicelluar cases, 4 got complete remission (CR), 24 got partial remission (PR),and the total effective rate was 55%,while in the parvicellular cases,5 cases got CR, 7 cases got PR,and the total effective rate was 75%.Conclusion The combination of bronchial artery perfusion and chemotherapy through alternating intravenous application of CAP and CE programs can promote the curative effect and reduce the side reactions in the treatment of lung cancers.
