基质金属蛋白酶-2、CD44 V6在胃癌中的表达及与肿瘤侵袭转移关系的研究

摘　　要：目的 :研究基质金属蛋白酶 2 (MMP2 )、CD44V6在胃癌中的表达及其与胃癌扩散转移的关系。方法 :应用免疫组织化学S P法观察MMP2和CD44V6在 71例胃癌组织中的表达。结果 :(1)MMP2在胃癌中表达明显高于正常胃组织 (P <0 .0 1) ;在肠型组、弥漫型组中的表达无显著差异 (P >0 .0 5 ) ;在淋巴结转移组表达明显高于无转移组 (P <0 .0 1) ;粘膜及粘膜下组表达与肌层组、浆膜层组相比差异非常显著 (P <0 .0 1)。 (2 )CD44V6在胃癌中表达明显高于正常胃组织 (P <0 .0 1) ;肠型组表达明显高于弥漫型组 (P <0 .0 1) ;在淋巴结转移组表达高于未转移组 (P <0 .0 5 ) ;粘膜及粘膜下组表达与肌层组、浆膜层组相比差异显著 (P <0 .0 5 )。 (3)胃癌MMP2和CD44V6两者表达间呈正相关 (r =0 .75 6 ,P <0 .0 5 )。结论 :MMP2、CD44V6在胃癌中均过度表达 ,MMP2、CD44V6均与胃癌的发生、侵袭和淋巴结转移有关 ,可作为预后的判断指标。Objective: Our aims were to study the expression of MMP2 and CD44V6 in gastric carcinoma and to investigate the relationship between the expression and metastasis of gastric carcinoma. Methods: The immunohistochemical method was used to study the expression of MMP2 and CD44V6 in 71 cases of gastric carcinoma. Results: (1) The positive rate of MMP2 expression in gastric carcinoma was higher than that in normal gastric tissue (P<0.01). There was no evident difference of MMP2 expression between intestinal type and diffusive type(P>0.05). The positive rate in lymph node metastasis group was evidently higher than that in non-metastasis group (P<0.01). The positive rate of MMP2 expression in mucosa and submucosa invasive group was much lower than those in muscular invasive group and serosa invasive group (P<0.01). (2) The positive rate of CD44V6 expression in gastric carcinoma was much higher than that in normal gastric tissue (P<0.01). The positive rate in intestinal type was evidentlyhigherthan that indiffusive type (P<0.01). The positive rate in lymph node metastasis group was much higher than that in non-metastasis group(P<0.05). The positive rate in mucosa and submucosa invasive group was much lower than that in muscular group and in serosa invasive group (P<0.05). (3)There was a positive correlation between the MMP2 expression and the CD 44 expression (r=0.756,P< 0.05). Conclusion: Both MMP2 and CD44V6 expressed too much in gastric carcinoma.They were related with the occurrence, invasion, and lymph node metastasis of gastric carcinoma.
