摘　　要：目的　探讨治疗囊性颅咽管瘤的有效方法。方法　总结采用经Ommaya化疗囊行瘤内化疗治疗囊性颅咽管瘤的经验。结果　 10例病人经初次置管抽液后视力、视野和高颅压症状很快改善 ,其他症状渐进性好转。置管后第 5dOmmaya化疗囊行造影检查 ,显示肿瘤囊壁完整 ,无造影剂外溢 ,开始行瘤内化疗 ,注射平阳霉素 8mg ,共 4～ 9次 ,平均 5次。出院时 6例复查CT ,显示囊肿消失 ,钙化部分吸收。无手术死亡率 ,无严重并发症 ,随防 3～ 49个月 ,结果满意。结论　经Ommaya化疗囊行瘤内化疗是治疗囊行颅咽管瘤的有效方法。Objective To evaluate the effective method for treating cystic craniopharyngioma. Method The experience of treatment of cystic craniopharyngioma with intratumoral chemotherapy through Ommaya reservior was summarized. Results After inserting Ommaya reservior and aspirating cystic fluid,the vision field of vision and symptoms of intracranial hypertension of all patients with cystic craniopharyngioma rapidly improved and other symptoms progressively turned better.Day 5 after inserting Ommaya reservior ,the injection of contrast medium through Ommaya reservior was performed.The result showed cystic wall intact, and no contrast medium leakage.Then intratumoral chemotherapy was undertaken, injecting pingyangmycin 8 mg,4～9 times,(average 7 times).On discharge,6 out of 10 patients were reexamined by head CT,revealing complete disappearance of the cyst and partial reduction in calcification .No death,serious complications and other side effects appeared.Eight patients were followed-up for 3～49 months.Good results had been gotten.Conclution lntratumoral chemotherapy through Ommaya reservior is an effective method for cystic craniopharygioma.
