增殖细胞核抗原表达与结直肠癌预后关系的研究

摘　　要：目的探索增殖细胞核抗原(PCNA)与结直肠癌的组织学类型、临床分期及预后的关系。方法收集外科手术切除的96例结直肠癌石蜡标本。采用LSAB免疫组织化学染色法检测PCNA;按阳性细胞所占比例分为1～4级。1、2级为PCNA弱级,3、4级为PCNA强级。结果(1)PCNA在肠癌细胞中的表达明显增多,并与肿瘤的分化程度有关,分化差的肿瘤细胞PCNA表达增多;(2)PCNA表达与Dukes分期及淋巴结转移有关。DukesC期及有淋巴结转移者,PCNA表达明显增多;(3)PCNA表达与患者的性别、年龄、肿瘤部位、组织类型等因素无关;(4)PCNA表达与术后复发率及死亡率有关,PCNA强级者术后复发率及死亡率均显著高于弱级者。结论PCNA在常规的病理切片上能反映细胞的增殖活性,是判断结直肠癌患者预后及指导临床治疗的一个较为理想的参考指标。Objective To investigate the relationship between the proliferating cell nuclear antigen(PCNA)and the histological type, clinical stages and prognosis of colorectal cancer.Methods Paraffin specimen of 96 cases with colorectal cancer were studied immunohistochemically with LSAB method for the expression of PCNA. Four grades were divided according to the proportion of positivity cells (1=0～25%2=26%～50%3=51%～75%4=76%～100%). Grade 1 and 2 of PCNA expression were defined as faint positivity . Grade 3 and 4 as intense positivity .Results (1) The expression of PCNA increased siginificantly in colorectal cancer cells. It was related to the degree of tumor differentiation.Increasing expression of PCNA occurred in poor differentiated tumor cells. (2) Expression of PCNA was related to Dukes stages and lymphatic metastasis.The expression of PCNA increased significantly in Dukes C stage and the cases with lymphatic metastasis.(3) The expression of PCNA was not correlated with the sex,age,location of tumor and histological type. (4)The expression of PCNA was correlated with the mortality and recurrence rate of the patients with colorectal cancer.Recurrence rate and mortality increased significantly in the group of PCNA intense positivity than that of faint positivity.Conclusions PCNA expression can reflect cell proliferating property,which is an ideal indicator to evaluate the prognosis and treatment of colorectal cancer.
