高分辨薄层CT扫描诊断外伤性视神经损伤

摘　　要：目的 :回顾性分析经手术证实的 2 9例外伤性视神经损伤的 CT表现 ,研究薄层 CT扫描对外伤性视神经损伤的诊断价值。方法 :CT扫描时采用层厚 2 mm,层间距 2 m m,对临床诊断为外伤性视神经损伤的 2 9例病人进行视神经和视神经管扫描。结果 :CT扫描发现视神经管骨折 15例 ,视神经周围血肿 3例 ,同侧蝶窦积血 11例 ,同侧筛窦积血 18例 ,视神经肿胀 8例。结论 :外伤性视神经损伤的 CT表现为视神经管骨折、骨折的间接征象和视神经损伤的本身征象。Objective: The CT findings in 29 cases of traumatic optic neuropathy confirmed by operation were retrospectively analysed, and studied the diagnostic value of thin section CT sacns to traumatic optic neuropathy. Methods: The nervus opticuses and optic canales of 29 patients to be diagnosed as traumatic optic neuropathy by clinical means were scaned using 2 mm axial scans with 2 spacing. Result: CT scans demonstrated that 15 cases with fractures of optic canal, 3 cases with hematomas around the nervus opticuses, 11 cases with hematocele of ipsolateral sphenoidal sinus, 18 cases with hematocele of ipsolateral ethmoidal sinus and 8 cases with optic swelling. Conclusion: The CT findings of traumatic optic neuropathy are the indirect signs of fractures of walls of optic canal and direct signs of traumatic optic neuropathy itself.
