摘　　要：目的　研究溃疡性结肠炎 (UC)患者的人类白细胞抗原 DR(HLA DR)基因分型 ,并分析其与抗中性粒细胞胞浆抗体 (ANCA)及疾病分型的关系。方法　用间接免疫荧光法分别对 80例UC患者和 12 3例健康者进行血清ANCA检测 ,同时用序列特异性引物 聚合酶链反应法 (PCR SSP)对HLA DR第 1到第 4个基因位点进行分析。结果　UC患者ANCA的阳性率为 5 5 .0 % ,对照组均为阴性 ;UC患者HLA DR2及DR15基因阳性率分别为 5 8.8%和 40 .0 % ,较对照组 30 .1%、17.9%显著增高 (P均<0 .0 5 ) ;与ANCA(- )UC患者比较 ,ANCA(+ )UC的DR15基因阳性率显著增加 ,而DR16基因阳性率显著降低 ;慢性持续型UCDR2和DR15基因阳性率较其他型显著增加。结论　HLA DR2和ANCA在UC中阳性率均显著增加 ,在ANCA阳性和阴性的UC中 ,分别由DR15和DR16基因阳性率增加所致 ;HLA DR基因分型与临床分型有关。Objective To study HLA DR genotyping and to analyze the relationship among HLA DR genotyping, antineutrophil cytoplasmic antibodies (ANCA), clinical classification of ulcerative colitis (UC). Methods Eighty UC patients, 123 healthy subjects (HS) were enrolled, and serum ANCA was detected with indirect immunoflorescence and HLA DR genotyped by polymerase chain reaction with sequence specific primers. Results Positive rate of ANCA in UC patients was 55.0%, but 0.0% in HS. HLA DR2 and DR15 gene positive rate in UC patients were 58.8% and 40.0% respectively, which were significantly higher than 30.1% and 17.9% in HS( P <0.05). When UC patients were divided into ANCA positive and negative groups, it is found that DR15 gene positive rate was significantly higher, but DR16 gene positive lower in ANCA positive group than those in ANCA negative group. HLA DR2 and DR15 gene positive rate in chronic persistent UC patients were significantly higher than those in other types of UC. Conclusions In China, the positive rate of HLA DR2 and ANCA is significantly increasing in patients with ulcerative colitis. The increase of DR2 gene positive rate is mainly resulted from the increase of DR15 gene positive rate in UC patients with ANCA positive, but from the increase of DR16 gene positive rate in UC patients with ANCA negative. HLA DR genotyping is associated with clinical classification of ulcerative colitis.
