摘　　要：目的　评价Ⅲ期中央型肺癌心包内处理血管全肺切除根治手术的效果。方法　对 42例Ⅲ期中央型肺癌行心包内处理血管全肺切除根治手术。结果　 3 8例手术切除 ,4例为探查。本组无手术死亡 ,手术后并发症发生率为 2 3 .7% ,1、3、5年生存率分别为 68.6% (2 4/ 3 5 )、3 7.1% (13 / 3 5 )、2 2 .9% (8/ 3 5 )。其中有 5例生存超过 7年 ,6例为无癌生存超过 5年 ,有 18例死于肺癌转移 ,9例死于其他系统疾病。结论　对Ⅲ期中央型肺癌行心包内处理血管全肺切除根治手术安全可靠 ,疗效显著。Objective To evaluate the effect of pneumonectomy deal with vessels in pericardium for central type lung cancer of stage Ⅲ.Methods Between 1990 and 1999,pneumonectomy deal with vessels in pericardium was performed in 42 pateints with central type lung cancer of stage Ⅲ.The postoperative effect and postoperatively complication were observed.Results Resection was performed in 38 patients and exploraton alone in 4 patients.There was no operative mortality,and the postoperative complication morbility was 23.7%.The 1,3 and 5-year survival rate was 68.6%,37.1% and 22.9%,respectively.7 patients have survived over 7 years.Conclusion Pneumonectomy deal with vessels in pericardium for treatment of central type lung cancer of stage Ⅲ is safe and effective.
