Hepatic Carcinomas: Value of Acquisition of Multiple Arterial and Portal Venous Phase Images at Dynamic Enhancement MR Imaging

摘  要:为评价磁共振 (MR)动脉期和门脉期动态成像对肝癌的检测价值 ,运用快速扰向梯度回波技术对 34例患者进行了 MR动态成像。在静脉团注 GD- DTPA的时间内连续采集 ,分别在动脉期与门脉期成像并与平扫 SE- T1像、 SE-T2像对肝癌的检测数目进行比较。结果显示 :6 4个得到证实的肝细胞癌 (HCC)中 ,平扫 SE- T1像发现 5 6个 (占 88% )、SE- T2像发现 5 4个 (84% )、动脉期成像发现 5 8个 (90 % )、门静脉期成像发现 46个 (72 % )。11个 HCC仅在平扫 SE序列显示 ,8个 HCC仅在动脉期得到确认 ,1个 HCC仅在门静脉期得到确认。动态成像对非 HCC肿瘤的检出率未增加。提示动脉期和门静脉期动态成像能检测出某些 SE平扫无法检出的 HCC肿瘤 ,是对常规To assess the value of examination by acquisition of multiple arterial and portal venous phase MR imaging in the diagnosis of hepatic carcinoma, dynamic MR imaging was performed in 34 patients by using a fast spoiled gradien echo technique. During intravenous bolus injection of GD DTPA multiple imaging was acquired. The number of tumors detected on arterial phase and portal venous phase images was evaluated and compared with un enhanced T1 and T2 weighted SE images. In the 64 hepatocellular carcinomas (HCCs), 48 (75 %) tumors were detected on un enhanced T1 weighted SE images, 46 (72 %) on un enhanced T2 weighted SE images, 54 (84 %) on arterial phase images and 44 (69 %) on portal venous phase images, respectively. Eight HCCs were detected only on arterial phase images, one only on portal venous phaseimage, and 11 only on un enhanced SE images. Detection of non HCC tumors did not increase by dynamic images. Acquisition of multiple dynamic arterial phase and portal venous phase images can detect some HCCs which cannot detect on conventional MR imaging and it is a complementary to routine MR imaging.
