摘 要:目的 探讨术前介入化疗对大肠癌细胞凋亡相关基因蛋白表达的影响。 方法 对2 0例大肠癌患者 ,借助于血管造影技术了解肿瘤的血供情况 ,发现肿瘤营养动脉 ,注入化疗药物 ;应用大肠镜分别于化疗前后及手术后取材。利用TUNEL法检测细胞凋亡情况 ,运用免疫组化方法检测PCNA、p5 3、Bcl 2、Bax、Fas基因。 结果 大肠癌细胞凋亡数 (积分光密度值 )化疗后 48h、5~ 7d分别为 1 2 7± 0 12、0 76± 0 0 7,均高于化疗前的 0 37± 0 0 6。癌细胞增殖活性积分光密度值 ,化疗后 48h为 0 40± 0 0 5 ,低于化疗前的 1 11± 0 11;化疗后 5~ 7d为 1 2 3± 0 0 8。大肠癌细胞p5 3、Bcl 2基因蛋白表达积分光密度值化疗后为 0 44± 0 0 2、0 6 0± 0 0 8,均低于化疗前的 1 0 9± 0 2 8、1 0 9± 0 0 7,而Bax、Fas基因蛋白表达积分光密度值化疗后 0 76± 0 0 7、1 6 9± 0 10均高于化疗前的 0 30± 0 0 3、0 39± 0 0 6。 结论 术前区域性动脉灌注化疗可以诱导大肠癌细胞凋亡 。Objective To investigate the effects of preoperative chemotherapy on apoptosis regulating genes of colorectal cancer. Methods This study was conducted in 20 patients with colorectal cancer. After the nutritional artery of the tumor was localized angiographically, chemotherapeutic drug was injected through the artery. Colorectal tissues were extracted with intestinal endoscopy before and after chemotherapy. Apoptotic cells were examined by TUNEL and expression of PCNA, p53, Bcl 2, Bax and Fas detected immunohistochemically. [WT5”HZ]Results[WT5”BZ] The number of apoptotic colorectal cancer cells (integral optical density) was 1 27±0 118, 0 76±0 067 and 0 37±0 056 in 48 and 144 hours after and before chemotherapy, respectively. The integral optical density of PCNA was 1 11±0 109, 0 40±0 051 and 1 23±0 082 before and in 48 hours and 5-7 days after chemotherapy. After chemotherapy, the density of the expression of p53 and Bcl 2 (0 44±0 02, 0 60±0 08) was lower than that before the treatment (1 09±0 28, 1 09±0 07). However, the density of the expression of Bax and Fas was increased after the treatment (0 76±0 07 vs. 0 30±0 03, 1 69±0 10 vs. 0 39±0 06). [WT5”HZ]Conclusions[WT5”BZ] Preoperative chemotherapy with local intra arterial infusion can increase the number of apoptotic cancer cells and affect the expression of apoptosis regulating genes in colorectal cancer.
