血清乳酸脱氢酶及同工酶对儿童急性淋巴细胞白血病的预后价值

摘　　要：目的 :探讨血清乳酸脱氢酶 (S -LDH)及其同工酶对儿童急性淋巴细胞白血病 (急淋 )的预后价值。方法 :测定和分析了 2 3例初诊儿童急淋和 7例急淋完全缓解期患儿S -LDH及其同工酶的变化。结果 :初诊急淋患儿S -LDH活性明显高于急淋完全缓解期患儿和正常对照儿童 (P <0 0 5) ,而急淋高危组S -LDH活性又明显高于急淋标危组 (P <0 0 5)。同时S -LDH同工酶谱显示 ,急淋高危组S -LDH同工酶 3(LDH - 3)的浓度显著高于急淋标危组 (P <0 0 2 )。结论 :高活性S -LDH和LDH - 3升高可作为儿童急淋高危因素之一 ,从而具有估计其预后的临床价值。Objective:To study the prognostic value of serum lactic dehydrogenase (LDH) and its isoenzymes in children with acute lymphoblastic leukemia (ALL).Methods:Serum LDH levels and its isoemzymes were measured and analyzed in 23 children with ALL at initial visit and 7 children with ALL who achieved completely remission. (ALL-CR).Results:The LDH levels have been found to be significantly higher in ALL at initial visit than those in ALL-CR and normal children (P<0 05) in our study.Furthermore,the LDH levels of patients in high-risk group were significantly higher than those of patients in standard-risk group (P<0 05).At the same time,the serum LDH isoenzyme patterns were analyzed.The concentration of LDH-3 isoenzyme was markedly higher in high-risk group than that in standard-risk group (P<0 02).Conclusions:The high LDH levels and LDH-3 concentration can be used as one of high-risk factors for ALL in children with.and have clinical prognostic value.
