低温保存人羊膜移植在重建眼表结构中的临床应用

摘　　要：目的 :评价保存人羊膜移植手术在眼表结构重建中的临床应用效果。方法 :对 8例不同眼表疾病患者采用低温保存人羊膜进行移植手术以重建眼表结构。其中 1例为眼球严重碱烧伤并发角膜溃疡 ,同时合并角膜缘干细胞严重缺乏 ,1例为假性翼状胬肉并发睑球粘连 ,1例为角膜血管翳伴有角膜上皮长期糜烂 ,4例为复发性翼状胬肉 ,1例为巨大翼状胬肉。结果 :重度碱烧伤并发角膜溃疡 ,同时因角膜缘干细胞严重缺乏 1例 ,术后 3个月羊膜植片全部溶解 ,手术失败。其余 7例均成功再造眼表结构 ,随访 3个月 ,无排异反应 ,植片透明生长 ,手术成功率为 87 5 %。结论 :保存人羊膜移植手术可有效重建眼表疾病的正常眼表结构。对合并角膜缘干细胞严重缺乏的病例 。Objective:To evaluated the efficacy of clinical application of transplantation of cryopreserved human amniotic membrane for ocular surface reconstruction.Methods:8 patients(8 eyes) with various ocular surface diseases were treated for ocular surface reconstruction with transplantation of cryopreserved human amniotic membrane:1 patient with severe alkali chemical burn combined with corneal ulcer and deficiency of limbal stem cell、1 patient with scar pterygium combined with symblepharon、1 patient with corneal vascularization and persistent corneal epithelial erosion,4 patients with recurrent pterygia,and last one patient with giant pterygium.Results:The patient with severe alkali chemical burn combined with cormeal ulcer and deficiency of limbal stem cell was treated unsuccessfully,as the amniotic membrane graft had been dissolved completely during of 3 months postoperative period.In the other 7 patients,ocular surfaces had been reconstructed successfully with transplantation of cryopreserved human amniotic membrane.During the period of 3 month follow up,the amniotic membrane had not been rejected,but grew transparent.The success rate was 87.5%.Conclusions:Cryopreserved human amniotic membrane can be used as a graft to reconstruct normal ocular surface.Transplantation of limbal stem cell should be performed simultaneously in cases combined with deficiency of limbal stem cell.
