高度近视白内障超声乳化及人工晶体植入的疗效

摘　　要：目的 :评价高度近视白内障超声乳化及人工晶体植入术的临床疗效。方法 :对眼轴 >2 6mm的 38例、5 2眼高度近视并发白内障患者 (Ⅰ级核 5眼 ,Ⅱ级核 2 1眼 ,Ⅲ级核 2 6眼 )行超声乳化及人工晶体植入术。患者平均 5 7 4岁 (5 2～ 73岁 ) ,平均眼轴长度 2 7 2± 1 5 6mm(2 6 1～ 2 9 2mm) ,平均人工晶体屈光度 (8 2 3±4 76 )D(+ 2～ + 15 )D。结果 :术后 1周裸眼及矫正视力≥ 0 3者 38眼 (73 1% )。并发症主要包括后囊膜破裂 2眼 (3 85 % ) ,轻度角膜水肿 7眼 (13 5 % )。术后视力差的主要原因是高度近视视网膜病变。结论 :超声乳化及人工晶体植入术是高度近视并发白内障患者的理想术式 。Objective:To assess the therapeutic effect of phacoemulsification with intraocular lens (IOL) implantation for cataract with high myopia.Method:Phacoemulsification with IOL implantation was done on 38 cases(52 eyes) suffered from complicated cataract and high myopia with long axial length(>26mm). The nucleus hardness was grade Ⅰ in 5 eyes, grade Ⅱ in 21eyes, grade Ⅲ in 26 eyes. The average age of the patients was 57.4 years (52～73 years).The mean axial length was 27.1±1.56mm(26.1～29.2mm). The mean diopter of IOL was (8.23±4.76)D (+2～+15D).Result:Postoperatively,the uncorrected or corrected visual acuity was 0.3 or better at 1 week in 38 eyes (73.1%) . There were mild corneal edema in 7 eyes and posterior capsular rupture in 2 eyes. The main reason of bad visual acuity was retinopathy with high myopia.Conclusion:Phacoemulsification with intraocular lens implantation is an ideal technique for cataract patient with high myopia. The technical skill might be of great importance in the success of the surgery.
