摘 要:目的 :探讨肾癌微血管密度 (MVD)与其组织学类型、肿瘤分级及转移之间的关系 ;评价MVD对肾癌预后估价的意义。方法 :应用Ⅷ因子相关抗原 (FⅧ -RAg)单克隆抗体进行免疫组化SABC法染色 ,检测 49例原发性肾癌根治性切除术标本中MVD。结果 :49例肾癌组织中平均MVD为 5 8 6 2± 2 3 86 ,明显高于正常肾组织 (P <0 0 1)。MVD与其局部淋巴结转移无相关性 (P <0 0 5 ) ,而与肾癌的分级及远处转移呈正相关 (P <0 0 1) ,与 5年生存期呈负相关 (P <0 0 1)。结论 :肾癌组织中MVD与其组织学类型、肿瘤分级、远处转移及生存期均密切相关 ,检测肿瘤组织中MVD可作为估介肾癌预后的一项重要指标。Objective:To investigate the relationship of MVD(microvessel density)in renal cell carcinoma to histological type,tumor grade,metastasis,and to evaluate the prognostic significance of MVD in renal cell carcinoma.Methods:Specimens were taken from 49 patients with primary renal cell carcinoma after radical nephrectomy.MVD was evaluated immunohistochemically using the labeled streptavidin-biotin-peroxidase method with a monoclonal antibody directed against factor Ⅷ-related antigen(F Ⅷ-RAg).Results:The mean MVD of 49 cases in renal cell carcinoma was 58.62±23.86,and was significanly higher than that in the normal renal tissue( P <0.01).MVD did not correlate with metastasis to local lymph node( P <0.05);however,MVD was positively correlated to tumor grade and distant metastasis( P <0.01),but negatively correlated to 5 year survival( P <0.01).Conclusion:It suggests that MVD in renal cell carcinoma is significantly correlated to histologial type,tumor grade,distant metastasis and survival.Assessment of tumor MVD is probably one of the most important prognostic feature in renal cell carcinoma.
