高度近视白内障摘出人工晶状体植入术

摘　　要：目的　探讨高度近视白内障摘出人工晶状体手术的临床效果。方法　 111眼高度近视白内障患者分成两组 ,一组行超声乳化人工晶状体植入术 ,另一组行现代白内障囊外摘出联合人工晶状体植入术。分别对两组术后视力 ,角膜散光及并发症进行对比分析。结果　术后 1周 ,超声乳化组及囊外摘出组最好矫正视力≥ 0 3者分别为 75 %和 69% ;平均角膜散光分别为( 1 2 5± 0 5 5 )D和 ( 3 72± 1 38)D ;术后并发症 :角膜水肿分别为10 %和 2 0 % ,后囊破裂分别为 5 %和 8% ,视网膜脱离发生率分别为 0 %和 2 85 %。结论　超声乳化人工晶状体植入术和白内障囊外摘出人工晶状体植入术均适于高度近视白内障患者 ,但前者具有散光小、反应轻、视力恢复快、手术并发症少等优点。Objective To approach the clinical effect of operation in high myopia cataract.Methods 111 eyes of high myopia cataracts are divided into two groups,Phaco/IOL are performed in group 1,Ecce/IOL in group 2.Postoperative vision,corneal astigmatism and complications are discussed in two groups.Results The best corrected vision ≥0.3 was in 75% and 69%,average corneal astigmatism in (1.25±0.55)D and (3.72±1.38)D,cornea swell in 10% and 20%,posterior capsule rupture in 5% and 8%,retinal detachment in 0% and 2.85% respectively in the two groups.Conclusions Both kinds of operations are equal fit to sufferers in high myopia cataract,but there is advance in astigmatism,vision,postoperative complications and reaction in group 1.
