三氧化二砷注射液对人体的毒副作用及其防治对策

摘　　要：目的　了解三氧化二砷 (As2 O3)注射液对人体的毒副作用 ,提出防治对策。方法　观察治疗前后体重 (Wt)及机体各系统变化 ,用原子吸收光谱法测定血、尿及发砷含量。结果　轻度不良反应 5 7 2 8% ,未停砷剂 ,未治或对症治疗 ,短期恢复 ;慢性中毒 3 71% ,其中有 2例 (0 93% )表现为重症肝病 ,均死于肝功衰竭。治疗后诱导缓解期病人 37 38%外周血WBC(PBWBC)升高 ,As2 O3 暂停或减量、小剂量化疗及WBC单采等 ,9 38%死于脑出血 ,90 6 2 %降至正常 ;治疗后缓解期病人5 2 16 %PBWBC减低 ,未停砷剂 ,给或不给生白药物 ,未见相关感染 ,逐渐恢复。结论　治疗剂量As2 O3 的不良反应多为轻度的、可逆性的 ,无需停As2 O3,对症可缓解 ,As2 O3Objective To study the main side effects and its prevention of intravenous injection of As 2O 3. Methods The changes of body weight and every systems of body were observed after treatment with As 2O 3 injection. The arsenic content in blood, urine and hair had been detected by atom absorbed-spectrum analysis.Results Their mild reactions were observed 57.28%and they could recovered after ceasing the arsenic treatment or allopathy. The chronic poisonings were 3.71%. The chronic moderate poisoning was 0.93% and all died of liver failure.As 2O 3 might cause the decrease of PBWBC of catabatic patients.There was no infection happened after allopathy treatment without ceasing drug.The results showed that As 2O 3 could distribute and discharge from the plasma without accumulation in blood.Conclusions The most of side effects of As 2O 3 were mild and convertible.Allopathy treatment could be effective to relieve the complications without stopping drug.As 2O 3 might be a safe and effective agent for most patients.
