口腔颌面部癌及癌旁组织端粒酶检测

摘　　要：[目的]研究口腔颌面部癌组织及相应癌旁组织中的端粒酶活性 ,探讨其作为口腔颌面部癌肿瘤标志物的可行性及其临床意义。[方法]用银染 TRAP法检测32例口腔颌面部癌组织及其相应的32例癌旁组织、4例混合瘤、4例乳头状瘤、2例造釉细胞瘤、5例正常口腔粘膜的端粒酶活性。[结果]32例口腔颌面部癌组织中 ,有30例端粒酶活性表达阳性 ,其阳性率为93.7%。32例癌旁组织中 ,有2例端粒酶表达阳性 ,其阳性率为6.3%。4例乳头状瘤、4例混合瘤、2例造釉细胞瘤、5例正常口腔粘膜端粒酶表达均为阴性。口腔颌面部癌组织端粒酶活性表达与该肿瘤的临床病理特征无明显相关性(P>0.05)。[结论]口腔颌面部癌组织中普遍存在端粒酶活性表达 ,而口腔良性肿瘤和正常口腔组织中端粒酶活性较少表达 ,端粒酶可作为诊断口腔颌面部癌的肿瘤标志物。癌旁组织端粒酶活化提示有少量肿瘤细胞 ,应随访以证实。To investigate the telomerase activity in human oral and maxillofacial cancer and corresponding adjacent tissues,to explore the possibility of telomerase as tumor marker and its clinicopathological significance.Thirty two oral and maxillofacial cancers and 32 adjacent peritumoral tissues,4 papillary tumors,4 mixed tumors,2 ameloblastomas,5 normal oral mucosas,were examined for telomerase activity by silver staining TRAP assay based on PCR.Thirty of 32 oral and maxillofacial cancers and 2 of 32 adjacent peritumoral tissue specimens were positive for telomerase activity with a positive rate of 93.7% and 6.3%,respectively.Telomerase activity was negative in 4 papillary tumors,4 mixed tumors,2 ameloblastomas,5 normal oral mucosas.There was no correlation between the clinical stage of oral and maxillofacial cancer and their expression of telomerase activity(P>0.05).[Conclusion]Positive telomerase activity occurs in the majority of oral and maxillofacial cancer examined. It can be used as a tumor marker in the diagnosis oral and maxillofacial cancer.The telomerase activation of the peritumoral tissue shows fewer tumor cells and should be followed up to be proved.
