泰素腹腔灌注化疗后患者腹腔液及动、静脉血浆中泰素含量的测定

摘　　要：目的 :测定泰素腹腔灌注后 ,妇科恶性肿瘤患者腹腔液、动、静脉血浆中泰素的含量及其临床意义。方法 :选择卵巢癌、输卵管癌伴有腹水者 10例 ,抽取腹水后 ,将泰素 135mg/m2 溶于生理盐水 15 0 0ml,注入腹腔 ,然后分别在 4、2 4、4 8h抽取腹腔液、动、静脉血各 2ml ,经分离、提取 ,采用紫外分光光度法检测泰素浓度。泰素腹腔化疗前 6、12h分别给予抗过敏药物治疗 ,并与顺铂 (CDDP) 6 0mg/m2 (或卡铂 30 0mg/m2 )联合应用。结果 :4、2 4、4 8h泰素浓度分别为 :腹腔液 7.96± 3.96mg/L、2 .14± 1.0 5mg/L、1.38± 0 .5 1mg/L ;静脉血浆浓度为 1.34± 0 .79mg/L、1.35± 0 .70mg/L、1.13± 0 .74mg/L ;动脉血浆浓度为 1.35± 0 .79mg/L、1.15± 0 .6 3mg/L、0 .90± 0 .6 4mg/L ,腹腔液浓度是动静脉血浓度的 10～ 13倍。毒性反应轻 ,5年随访总有效率为 6 0 .6 7%。结论 :泰素腹腔灌注化疗局部浓度高 ,清除速率慢 。Objective:To detect taxol contents of the abdominal cavity fluid,arterial and venous plasma after intraabdominal cavity perfusion in patients with gynecologic malignant tumors and their clinical significance.Methods:10 cases of ovarian cancer and fallopian tube cancer with ascites were selected entered for the study,taxol 135mg/m 2 in normal saline 1 500ml were injected into abdominal cavity after withdrawing entirely ascites,then drew the abdominal cavity fluid,arterial blood,venous blood each of 2ml at 4h,24h,48h after injection.The contents of taxol were detected by ultraviolet spectrophotography.At 6h and 12h before taxol intraabdominal cavity chemotherapy,antiallergic drugs and cisplatin 60 mg/m 2 (or carboplatin 300 mg/m 2) were used combinely.Results:The mean concentrations at 4h,24h and 48h were 7.96±3.96mg/L,2.14±1.05mg/L,1.38±0.51mg/L respectively in cascites;1.35±0.79mg/L,1.35±0.70mg/L,1.13±0.74mg/L respectively in venous blood;1.35±0.79mg/L,1.15±0.63mg/L,0.90±0.64mg/L respectively in arterial blood.The toxic reaction was mid.After five-year following up the total reactive rate was 60.67% . Conclusions:The chemotherapy of taxol intraabdominal cavity perfusion has high blood concentration,low clearance rate and high therapeutic effectiviness.Thus,it benefits for killing abdominal cavity malignant tumors.
