摘  要:目的 探讨利用CD4 4V6,nm2 3 H1及肿瘤分化程度联合预测结直肠癌同期或近期肝转移。方法 运用免疫组化SABC法测定CD4 4V6,CD44s,nm2 3 H1在癌组织的表达 ,同时记录有关的临床病理指标。结果 结直肠癌有同期或近期肝转移者癌组织中CD4 4V6,nm2 3 H1表达较无肝转移者有显著差异 (P <0 .0 5 ) ,利用Logistic回归分析得预测同期或近期肝转移方程 :Logit(p) =- 4.334 2 +2 .6 776X5- 2 .5 112X7+2 .6 85 1X11式中X5为CD4 4V6,X7为nm2 3 H1,X11为分化程度。预测符合率 94.7%。结论 利用回归方程计算同期或近期肝转移风险值大于 0 .5则转移可能大 ,小于0 .5时则转移可能性小。CD4 4V6,nm2 3 H1及肿瘤分化程度等三项指标可以较准确地预测结直肠癌同期或近期肝转移风险 ,为外科决策提供依据。Objective To predict whether a patient with colorectal carcinoma is suffering from short-term or concurrent liver metastases by multivariates including the gene expression of CD_ 44V_6,nm23-H_1 and the degree of differentiation.Methods The expressions of CD_ 44V_6,CD_ 44s,nm23-H_1 were detected using immunohistochemistry in 114 colorectal carcinomas and the clinicopathological characteristics were recorded.Results There was significant difference in the expression of CD_ 44V_6 and nm23-H_1 between liver metastases and without liver metastases.In multivariate analyses using Logistic regression, an equation of predicting the risk of short-term or concurrent liver metastases was gained (Logit(p)=-4.3342+2.6776X_5-2.5112X_7+2.6851X_ 11,X_5,X_7,X_ 11 representing CD_ 44V_6,nm23-H_1,degree of differentiation respectively).The coincidnce was 94.7%.Conclusion The patients with P value exceeding 0.5 in the equation are at an increased risk of short-term or concurrent liver metasases.Therefore it facilitates surgical decision-making.
