多药耐药基因在滋养细胞肿瘤组织中的表达及其临床意义

摘　　要：目的　探索MDR 1(multidrugresistance)基因在滋养细胞肿瘤组织及正常组织中的表达情况及与化疗、临床期别等关系。方法　运用RT PCR方法对 2 1例恶性滋养细胞肿瘤及 5 0例正常绒毛、胎盘的多药耐药基因表达进行检测分析 ,同时用免疫组化方法对 6例绒毛、胎盘和 12例化疗后恶性滋养细胞肿瘤糖蛋白的存在状态进行探讨。结果　正常绒毛、胎盘MDR 1mRNA阳性表达率分别为 2 4 0 % (6 /2 5 )、32 0 % (8/2 5 ) ,两者无明显差异性 (P >0 0 5 ) ;未化疗病人无MDR 1基因阳性表达 ,化疗后病人 33% (4/12 )MDR 1mRNA阳性 ;阳性率与临床期别有关 ,Ⅰ期阳性率为 0 ,Ⅱ期阳性率为 1/5 ,Ⅲ期阳性率为 3/10 ;糖蛋白在细胞滋养细胞、合体滋养细胞及过渡型细胞中的分布无明显差别。结论　滋养细胞肿瘤MDR 1基因表达的变化 ,为化疗方案的调整提供了理论依据。Objective To study the expression of multidrug resistance(MDR) 1 gene in malignant trophoblastic tumor tissue and the connection between chemotherapy and different clinical stages.Methods Experiments and analysis were done by using RT PCR technique on 21 cases of malignant trophoblastoma,50 cases of normal villi and placenta for MDR and at the same time the determination of 12 cases of malignant trophoblastic tumor,6 cases of villi and placenta glycoprotein were made for their existing states by using immunohistochemical method.Results 24%(6/25)normal villi tissue and 32%(8/25)normal placenta tissue were MDR 1 mRNA postitive and there was no significant difference(P>0\^05).There was no MDR 1 gene positive expressed(0/9)in nonchemotherapeutic patient while 33%(4/12)MDR 1 mRNA showed positive.Positive rate was associated with clinical stages.In stage Ⅰ,the positive rate was 0,while in stage Ⅱ,it raised to 1/5 and in stage Ⅲ,it appeared to be 3/10.Distribution of glycoproteins among the cytotrophoblast,syncytrophoblast and transitional cells showed no significant difference.Conclusion Expression of MDR 1 gene of malignant trophoblastic tumor can supply a theoretical basis for adjusting chemotherapeutic protocol.
