抗青光眼术后并发白内障的超声乳化摘除术

摘　　要：目的 :探讨抗青光眼术后白内障的超声乳化术的手术技巧 ,并评价手术效果。方法 :对 41例 (45眼 )抗青光眼术后并发白内障的患者 ,行经颞侧透明角膜隧道切口的白内障超声乳化摘除和折叠式人工晶体植入术。结果 :除 3眼术后第一天出现一过性眼压升高 ,其余术眼术后眼压均有所下降 ,但与术前比较无统计学差异。随访时矫正视力≥0 .5者 40眼 (占 89% ) ;视力≤ 0 .1者 2眼 (占 4% ) ,为绝对期青光眼患者。角膜内皮丢失率为 10± 3.0 %。手术并发症主要是后囊破裂、晶体悬韧带部分离断和虹膜损伤。结论 :超声乳化吸除联合折叠式人工晶体植入术是治疗抗青光眼术后白内障的有效方法之一 。Objective: To evaluate the operation of phacoemulsification for complicated cataract after glaucoma surgery. Methods: The phacoemulsification through tunnel incision and foldable IOL implantation were run in 41 cases (45 eyes) of complicated cataract after glaucoma surgery. Results: In 3 eyes, IOP was temporary high on the first day after surgery. In the other eyes, compared to preoperative IOP, the postoperative IOP was slightly low, but there was no statistical difference. During following-up, there were 40 eyes (89%) that got corrected vision better than 0.5 and 2 eyes (4%) of absolute glaucoma in which the visual acuities were no more than 0.1. The percentage of corneal endothelium loss was 10 ( 3.0%. The major complications were rupture of posterior capsule, rupture of partial zonules and damage of iris. Conclusions: In phacoemulsification and foldable IOL implantation for complicated cataract after glaucoma surgery, it must be taken into account to protect corneal endothelium and prevent postoperative IOP rising.
