乳腺癌术前化疗的临床研究

摘　　要：目的　探讨术前化疗对可手术乳腺癌的远期疗效。方法　将可手术的乳腺癌患者 71 6例分为两组 ,术前化疗组 (A组 ) 3 51例 ,未术前化疗组 (B组 ) 3 65例。A组术前化疗 ,每周 1次共 2～ 4次 ,化疗后 2周内行根治手术。两组患者术后 2周行辅助性化疗。结果　 (1 )化疗有效率 ,CR +PR 72 3 5% ;(2 )Ⅲ期患者 ,A组 5年和 1 0年生存率 (OS)和无病生存率 (DFS)均明显高于B组 (P<0 .0 5) ;(3 )Ⅱ期乳腺癌患者 ,5年OS及DFS与B组无统计学意义 (P >0 .0 5) ,但 1 0年OS及DFS则明显高于B组 (P <0 .0 5)。结论　术前化疗可缩小肿瘤 ,增加手术切除率 ,减少复发、转移 ,并可提高患者的远期疗效。Objective To probe into the forward curative effect of the preoperative chemotherapy used for breast cancer patients.Methods 716 cases of breast cancer patients are divided into two groups,Group A preoperative chemotherapy group (351 cases) and Group B non chemotherapy group (365 cases).Group A receives preoperative chemotherapy,once per week,2～4 times intotal.After chemotherapy the patients of the two groups receive auxiliary chemotherapy two weeks after operation.Results (1)The effective rate of chemotherapy:CR+PR 72.35%;(2)The stage Ⅲ patients:The 5 year and 10 year overall survival rate (DFS) of Group A are evidently higher than Group B (P<0.05);(3)The 5 year DS and DFS stage Ⅱ breast cancer patients have no statistical significance,just like Group B(P>0.05),but 10 year DS and DFS in Group A is edvidently higher than that of Group B.Conclusion The preoperative chemotherapy can reduce tumors increase the excixion rate,it can also reduce relapse metastasis meanwhile it can increase the forward effective result of patients.
