氟脲脱氧核苷,顺铂配合外照射治疗鼻咽癌的临床研究

摘　　要：目的　观察比较氟脲脱氧核苷 ( FUDR)或氟脲嘧啶 ( 5 - FU)联合顺铂 ( DDP)配合外照射治疗鼻咽癌的近期疗效和急性毒副反应。方法 46例 期以上有颈部淋巴结转移病灶的鼻咽癌患者被随机分为 FUDR组和 5 - FU组。放疗前分别给予 FUDR1 0 0 0 mg/日 ,DDP2 0 mg/日或 5 -FU1 0 0 0 mg/日 ,DDP2 0 mg/日 ,静脉滴注 ,连续 5天。休息 3天后开始放疗 ,两组放疗剂量和方法相同。结果两组患者的总有效率 ( CR+ PR)分别为 96% ( FUDR组 )和 91 % ( 5 - FU组 ) ,无显著差异。重度口咽炎的发生率 FUDR组为 30 % ,5 - FU组为 65 % ( P<0 .0 5 ) .结论 FUDR加 DDP与 5 - FU加 DDP用于鼻咽癌诱导化疗相比较具有毒副作用小 ,患者易耐受的优点。Objective To compare the response and side effects of FUDR plus DDP With 5\|FU plus DDP fallowed by radiotherapy to treat patients with locally advanced nasopharyngeal carcinoma.Methods 46 patients with neck metastasis nasopharyngeal carcinoma were randomized into two groups:FUDR group or 5\|FU group.FUDR 1000mg or 5\|FU 1000mg plus DDP 20mg iv on day 1～5 for patients before radiotherapy.3 days after chemotherapy all patients received the same radiotherapy.Results 4 weeks after treatment,the effective rates(CR+PR)were 96% in FUDR group and 91% in 5\|FU group and was no significant difference between the two groups.However,during the treatment,patients with severe acute oral mucositis in FUDR group were less than that in 5\|FU group.Conclusion FUDR can be used as neoadjuvand chemotherapy for patients with locally advanced nasopharyngeal carcinoma,which has less side effect and is easily tolerable.More studies are necessary for long\|term result.
