VEGF表达和血管生成与胃癌的浸润、转移及预后的关系

摘　　要：目的　探讨血管内皮生长因子 (VEGF)表达和血管生成与胃癌的浸润、转移及预后的关系。方法　材料取自安徽医科大学第一附属医院普外科 1994年 10月～ 1997年 10月手术切除标本 73例。小鼠抗人VEGF单克隆抗体与小鼠抗人Ⅷ因子相关抗原单克隆抗体用于检测肿瘤组织中的VEGF蛋白和微血管 (MV)。结果　 73例胃癌组织中 ,VEGF阳性表达率为 5 2 1% (38/ 73)。VEGF阳性表达的肿瘤的微血管计数 (MVC)高于阴性者 (P <0 0 1)。VEGF表达和MVC与肿瘤的浸润深度、分期、淋巴结转移、脉管侵犯(血管侵犯、淋巴管侵犯 )、远处转移具有明显相关性 (P <0 0 1)。VEGF阳性表达者复发率高于阴性者 ,VEGF阳性者预后较阴性者差。Cox s比例危险模型分析表明 :VEGF是一个独立的预后因子。结论　VEGF通过刺激新生血管生成 ,进一步促进肿瘤的浸润、转移。Objective To investigate the expression of vascular endothelial growth factor (VEGF) and neovas cularization in gastric cancer and their relationships between tumor invasion, metastasis and prognosis. Methods 73 specimens from patients with gastric cancer were collected from the affiliated hospital of Anhui medical university from 1994 to 1997. The expression of VEGF and microvessel (MV) were detected by using anti VEGF monoclonal antibody and anti vWF (von Willebrand Factor) monoclonal antibody. Results Thirty eight (52 1%) cases were positive for VEGF. Microvascular count, determined by immunostaining for vWF, was significantly higher in VEGF positive tumors than that in VEGF negative ones. ( P< 0 01). The expression of VEGF and MVC were significantly related with the depth of tumor invasion, lymphatic invasion, vessel invasion, lymph node metastasis and remote metastasis ( P< 0 01). Moreover, patients with VEGF positive expression had a significantly higher recurrence and poor prognosis than those with VEGF negative expression. Multivariate analysis by Coxs proportional hazards model showed that the expression of VEGF was an independent prognostic factor in patients with gastric cancer. Conclusion VEGF may promote the invasion, metastasis and recurrence of the tumor by stimulating angiogenesis.
