表麻下白内障超声乳化手术200例报告

摘　　要：目的 :分析表麻下进行超乳手术的安全性及优缺点。方法 :回顾性总结 2 0 0例表麻下白内障超声乳化吸出术的术中配合情况、并发症及术后视力情况。结果 :手术时间 15分以内者 ,病人的耐受性普遍较佳。并发症 :术中后囊破裂 5例 ,人工晶状体 2例囊袋内植入 ,3例睫状沟植入。术后角膜内皮斑片状混浊 6例 ,在一周内消退。术后视力 :16 0例无明显眼底病变者 ,术后 1天和 1周裸眼或矫正视力≥ 0 .5者分别为 90 %和10 0 %。 14例视力不佳者为青光眼晚期、老年性黄斑变性或高度近视黄斑病变患者。结论 :在熟练掌握超乳技术后 ,表麻下超乳手术是安全。Objective:To analyze the safety, benefits and drawbacks of phacoemulsification under topical anesthesia.Methods:In this retrospective study, 200 cases received phacoemulsification under topical anesthesia. The patients cooporation during surgery, complications during and after surgery and visual acuity after surgery were analyzed.Results:Cooperatiion was good in cases where surgery time was less than 15 minutes. The posterior capsules were broken in 5 cases, intracapsular IOLs were implanted for 2 cases and in the sulcus for 3 cases. Six cases had corneal endothelium edema after surgery but this cleared within a week. For visual acuity or best corrected visual acuity in 106 cases without retinal disease, 144 cases (90%) had better than 0.5D on the first day and all (100%) were better than 0.5D within a week. Of the other 14 cases, visual acuity was under 0.5D due to glaucoma, age related macular degeneration or myopia macular degeneration.Conclusion:Once the technique is mastered, it is safe to perform phacoemulsification under topical anesthesia.
