摘　　要：目的　探讨中细胞周期素D1(cyclinD1)和p16蛋白的表达与鼻咽低分化鳞癌预后的关系。方法　应用免疫组化SP法检测cyclinD1和p16在 66例鼻咽低分化鳞癌和 2 0例鼻咽部炎症黏膜组织中的表达。分析这两种蛋白与鼻咽癌颈淋巴结转移、临床分期、复发和预后的关系。结果　p16的缺失表达在有淋巴结转移组高于无转移组(P <0 0 5 ) ,而cyclinD1的过表达与淋巴结转移无关 (P >0 0 5 )。cyclinD1的过表达在有复发组高于无复发组(P <0 0 5 ) ,而p16的缺失表达与复发无关 (P >0 0 5 )。p16的缺失表达和cyclinD1的过表达均与临床分期无关(P >0 0 5 )。p16的缺失表达和cyclinD1的过表达小于 5年生存组均明显高于大于 5年生存组 (P <0 0 5 )。结论　p16的缺失表达可能与淋巴结的转移和预后有关 ;cyclinD1的过表达可能与复发和预后有关。Objective To evaluate the relati onship bwteeen expression of cyclin D1 and p16 protein and prognosis of poorly differentiated nasopharyngeal squamous cell carcinoma (NPC). Methods Cyclin D1 and p16 expression in 66 cases of N PC a nd 20 cases of inflammatory nasopharyngeal mucosa was defected using SP immunoh istochemical m ethod. The results was correlated with lymph node methastasis, clinical stage, recurrence and prognosis of the patients. Results Loss of p16 in patients with lymph node metastasis was higher than in pa tients without lymph node metastasis(P<0.05). The overexpression of cylin D1 was not correlated to metastasis (P>0.05). Overexpressi on of cyclin D1 was hi gher in recurrent than in nonrecurrent tumors (P<0.05). Loss of p16 wa s not correlated to recurrence(P>0.05). No significant relationship was f ound between loss of p16 and overexpression of cyclin D1 and the clinical s tage in N PC (P>0.05). The loss of p16 and the overexpression of cyclin D1 were more frequent in tumors with <5 year surival than those with >5 year surival ( P<0.05).Conclusion Loss of p16 may be involved in tu mor metastasis. Overexpression of cyclin D1 may be involved in tumor recurr ence. p16 and cyclin D1 may be prognostic indicators in NPC.
