摘 要:目的 探讨早期糖尿病视网膜病变 (diabetic retinopathy,DR)患者视网膜电图 (electroretinogram ,ERG)振荡电位 (oscillatory potentials,OPs)和色觉监测的指标。方法 尚无 DR改变 (DR0期 )的糖尿病患者和DR1期患者共 70例 (134眼 )分别用 Neuropack- 电生理仪测试 ERG OPs及用 FM- 10 0 Hue色相子排列法检查色觉 ,并与 30例 (5 9眼 )正常人的测试结果相比较。两种检查的测试结果亦作比较分析。结果 糖尿病组 OPs各小波潜伏期延长 ,振幅及各子波振幅总和下降 (P <0 .0 5 ) ,异常率为 40 .3%。除 OP3振幅外 ,DR1期组各波振幅及振幅总和低于 0期组 (P <0 .0 5 )。色觉异常率为 39.6 % ,轴向分析表明全部为蓝色觉异常 ,1期患者比 0期的平均错误分高 (P <0 .0 5 )。二者检查结果有良好一致性 (x2 =12 .1,P <0 .0 0 5 ) ,但检出率无显著差异 (x2 =0 ,P >0 .0 5 )。结论 ERG OPs和色觉检查可作为评价患者视功能及 DR进展的指标 ,有利于对糖尿病患者进行跟踪随访及 DR的早期防治 ,具有一定的临床应用价值。Objective To investigate the datum of electroretinogram(ERG) oscillatory potentials(OPs)and color vision in patients with diabetic retinopathy(DR) in early stage.Method 70 patients (134 eyes) who were without DR or Ⅰ stage were enrolled in the study.ERG OPs was detected by Neuropack-Ⅱ electrophysiological recording systems and color vision function was measured by the Farnsworth-Munsell 100-hue test.The results were compared with those of 30 normal subjects (59 eyes).The sensitivities of the two examinations were also analyzed.Results The implicit time of OPs wavelets prolonged and the amplitude or summed amplitude reduced in diabetes mellitus (DM) group (P<0 05).The abnormal rate of OPs was 40.3%.Except OP 3,the amplitude and summed amplitude of the wavelets were lower in patients with DR Ⅰ stage than that with 0 stage (P<0 05).The abnormal rate of color vision was 39.6% and the analysis of color axis indicated the abnormal color is blue.The average error score was higher in patients with DR Ⅰ stage than that with 0 stage (P<0 05).Both of the examinations were consistent with each other (x 2=12.1,P<0 005),but the abnormal rate has no difference (x 2=0,P>0 05) Conclusion ERG OPs and color vision function can evaluate vision and predict the progression of DR.They have clinic value to follow patients with DM and prevent DR in early stage.
