椎管内表皮样囊肿的MRI诊断

摘　　要：目的:评价MRI对椎管内表皮样囊肿的诊断。材料与方法:搜集24例经手术病理证实的椎管内表皮样囊肿,重点分析其MRI特点及发病部位。结果:本组病例中,病人年龄最小3岁,最大42岁,平均157岁。本组病例发病部位1例位于硬膜外,16例位于髓外硬膜下,7例位于脊髓内,病灶位于胸段3例,脊髓圆锥内7例,脊髓圆锥水平髓外硬膜下3例,马尾11例。其MRI信号表现为T1WI低信号13例,等低信号9例,等高信号2例;T2WI高信号15例,等高信号7例,低信号1例,等信号1例。在22例增强扫描的病人中,5例病灶边缘呈轻度增强,17例增强扫描病灶无变化。3例病灶边缘可见脂肪信号。14例合并有先天畸形。结论:椎管内表皮样囊肿MRI表现较具特征性,增强扫描无变化或呈病灶边缘轻度增强,发病多见于儿童及青少年,多伴有先天畸形。除与皮样囊肿鉴别困难外,可与其它椎管内肿瘤鉴别。Objective:To evaluate MRI diagnostic value of e pidermoid cyst in the spinal canal.Materials and Methods :Twen-ty -four patients(3～42years old)with pathologically proven epidennoid cyst of spinal canal were studied.MRI features of these tu-mors were analysed with emphasis on s ignal characteristics and location.Results:The tumors showed low signal(13/24),iso -low signal(9/24)and iso -high signal(2/24)on T 1 WI;the tumors showed high signal(15/24),iso -high signal(7/24),low signal(1/24)and iso -signal(1/24)on T 2 WI.On contrast enhanced studies,th e tumors showed slight peripheral en hancement (5/24)or no contrast enhancement (17/24).Fat signal was found in three cases.Most of the tumors were located aroun d the conus and cauda e-quina,three cases located in subdur al space of thoracic section;16case s located in subdural space,seven ca ses located within the spinal cord,one case located in epid ural space.Fourteen cases complica ted with congenital deformation.Conclusion:The epidermoid cyst in the spinal canal most likely o ccurred around the conus region and c auda equina.Most of them located in s ubdural space.The majority of the tumors showed low or i so -low signal on T 1 WI and high or iso -high signal on T 2 WI.The tumors demonstrated slightl y peripheral enhancement or no contra st enhancement.Some cases are compl icated with congenital deformation.The epidermoid cyst could be distinguished from other tu mors of the spinal canal expect dermo id cyst.
