腹膜后纤维化引起的肾积水11例报告

摘　　要：目的 :提高腹膜后纤维化的诊断及治疗水平。方法 :对 1986年 2月～ 1999年 11月期间所收治的 11例腹膜后纤维化引起的肾积水患者的临床资料进行回顾性分析 ,其中原发性者 6例 ,继发性者 5例 ;3例行输尿管内支架治疗 ,8例行手术治疗并于术后加用激素治疗。结果 :随访 6～ 38个月 ,术后肾功能均有改善或恢复正常 ;6例原发性者预后良好 ,3例继发于恶性肿瘤者于术后 3～ 8个月死亡 ,另 2例血沉降至正常。结论 :对于腹膜后纤维化引起的肾积水 ,逆行肾盂造影、B超、MRI等检查可明确诊断 ,宜尽早采取手术等措施解除梗阻 ,引流尿液是有效的治疗方法。Purpose:To present and evaluate the diagnosis and treatment of 11 cases of retroperitoneal fibrosis causing hydronephrosis.Methods:A retrospective study from Feb.1986 up to Nov.1999,for the treatment of 11 cases of retroperitoneal fibrosis causing hydronephrosis has been made.Results:The retroperitoneal fibrosis was idiopathic in 6 and secondary in 5.Surgical intervention was undertaken in 8 with postoperative administration of corticosteroids and intraluminal stent was inserted in 3.The patients have been followed up for 6 to 38 months with improved or normal renal function and the prognosis of primary retroperitoneal fibrosis has been good while it was very poor in fibrosis secondary to malignancy,3 being already dead.Conclusions:Retrograde urogram, Ultrasonography and MRI are extremely helpful. To relieve the obstruction by surgical intervention is very effective.
