摘 要:目的 了解输尿管移行细胞癌组织中bcl 2和TGF β1 蛋白的表达。方法 采用免疫组化技术 ,检测 5 2例输尿管移行细胞癌与 1 1例正常输尿管移行上皮细胞中bcl 2和TGF β1 蛋白的表达。结果 1 1例正常输尿管中bcl 2蛋白表达阳性 2例 (1 8.2 % ) ,TGF β1 蛋白表达阳性 1 1例 (1 0 0 % )。 5 2例输尿管移行细胞癌中bcl 2蛋白表达阳性 2 0例 (3 8.5 % ) ,TGF β1 蛋白表达阳性 2 6例 (5 0 % )。bcl 2阳性率在高、中、低分化癌中分别为 2 1 .1 %、3 6 .4%和 72 .7%。在TiS~T1 和T2 ~T4期阳性率分别为 3 1 .6 %和5 7.1 %。TGF β1 阳性率在高、中、低分化癌中分别为 73 .7%、45 .5 %和 1 8.2 %。在TiS~T1 和T2 ~T4期阳性率分别为 6 0 .5 %和 2 1 .4%。在输尿管移行细胞癌中bcl 2表达程度随肿瘤分级增加而增加 ,在各级肿瘤分化间差异有显著性 (χ2 =7.93 ,P <0 .0 5 )。浅表性癌bcl 2表达阳性率低于浸润性癌 ,但差异无显著性 (χ2 =2 .82 ,P >0 .0 5 )。随肿瘤分级增加 ,TGF β1 阳性率下降 (χ2 =8.90 ,P <0 .0 5 ) ,随肿瘤由浅表型向浸润型转化 ,阳性率下降 (χ2 =6 .2 6 ,P <0 .0 5 ) ,差异均有显著性。结论 bcl 2和TGF β1 参与了输尿管移行细胞癌的发生发展过程 ,可作为评估其生物行为的瘤标。Objective To study the role of bcl 2 and TGF β 1 protein expression in ureteral transitional cell carcinoma.Methods The level of bcl 2 and TGF β 1 protein expression was investigated by immunohistochemistry in 11 cases of normal ureter and 52 cases of ureteral transitional cell carcinoma.Results bcl 2 was positive in 2 of 11(18.2%) and TGF β 1 being positive in 11 of 11(100%) normal ureters.bcl 2 was positive in 20 of 52(38.55%) and TGF β 1 being positive in 26 of 52(50%) tumors.The bcl 2 positive rate in high,middle and low differentiation of tumors was 21.1%,36.4% and 72.7% respectively and in stage Tis~T 1 and T 2~T 4 tumors being 31.6% and 57.1%.The TGF β 1 positive rate in high,middle and low differentiation of tumors was 73.7%,45.5% and 18.2% respectively and in stage TiS~T 1 and T 2~T 4 tumors being 60.5% and 21.4%.Positive rate for bcl 2 significantly increased with the progression of the pathological (P<0.05),whereas in clinical stages there was no significant difference (P>0.05).Positive rate for TGF β 1 decreased with the progression of the pathological grade and clinical stage (P<0.05).Conclusion bcl 2 and TGF β 1 may play a role in the development of ureteral transitional cell carcinoma and may serve as a tumor marker to evaluate the biological behavior of ureteral transitional cell carcinoma.
