腹腔灌注羟基喜树碱治疗晚期肝癌腹水的临床研究

摘　　要：目的　观察羟基喜树碱 (HCPT)对晚期肝癌所致恶性腹水的疗效。方法　腹腔穿刺抽出腹水 ,然后腹腔内注射HCPT(2 0～ 30mg) ,并同时静脉输血及白蛋白 ,每周 1次 ,共用 2周。结果　全组 30例 ,完全缓解 (CR) 3例 ,部分缓解 (PR) 17例 ,总有效率为 6 6 6 7% ,毒副反应主要是白细胞下降 ,发生率2 7 2 8% ,白细胞毒性均为Ⅰ°。结论　HCPT腹腔内灌注治疗晚期肝癌腹水疗效满意 ,能改善其生活质量。Objective The clinical efficacy of HCPT for malignant ascites of advanced hepatic carcinoma was summarized.Methods Intraperitoneal perfusions of HCPT(20～30mg) were given in 30 patients after drawing ascites, then blood and albumin were infused in vein, once a week in two weeks.Results Three patients were CR and 17 patients PR in 30 patients, and a total effective rate was 66.67%. The main side effect was leucocytopenia in three patients(27.28%) within Ⅰ° leucocyte toxicity.Conclusion Intraperitoneal HCPT is effective for malignant ascites of adavanced hepatic carcinoma and benefited to improved quality of life.
