摘  要:目的 :研究核DNA数控定量分析技术对角结膜缘上皮性肿物病理诊断的意义 ,特别是进行早期组织病理学改变的诊断价值。方法 :应用自动图象分析技术及定量病理学方法 ,对 5 1例角结膜缘上皮性新生物 (上皮良性增生、上皮非典型性增生、原位癌、鳞状细胞癌 )定量测定核DNA含量和细胞核仁组成区嗜银蛋白 (argentumofnuclearorganizerregion ,AgNOR)含量。结果 :在四种不同病变中 ,细胞核DNA倍体含量 (2C ,3~ 4C ,≥ 5C)存在着显著的差异 ;AgNOR颗粒的分布形态、颗粒大小及所占核仁面积大小均存在着明显的差异 ,与病变的良、恶程度以及细胞的分化状况有着密切的关系。结论 :应用自动图象分析技术对角结膜缘上皮性肿物进行核DNA及AgNOR含量数控定量分析测定 ,可作为对组织病理学诊断可靠的辅助诊断方法 ,进一步分析了解增殖性病变的细胞增殖活性 ,评价其早期增殖分化潜势 ,鉴别良恶性病变 ,有利于从细胞生物学特点评估患者的预后 ,具有一定的实用意义。Objective:To study the significance of nuclear DNA and argentum of nuclear organizer region (AgNOR) content,observed with automatic image analysis technology(AIAT), on the pathologic diagnosis,especially the early pathologic changes of limbal neoplasms.Methods:AIAT and quantitative pathologic methods were used to measure the DNA ploidy contents and AgNOR contents in 51 specimen blocks of various limbal neoplasms (benign epithelial hyperplasia 19,atypical hyperplasia 6,carcinoma in situ 14,squamous cell carcinoma 12).Results:There were significant differences of nuclear DNA ploidy contents (2C,3~4C,≥5C) among the four different neoplasms and also in the distributive pattern,size and area of AgNOR granules.All these features had close relationship to the condition of differentiation of cells and the degree of malignancy.Conclusion:In limbal epithelial neoplasms,AIAT is an effective method to measure nuclear DNA and AgNOR contents of tumor cells, and by analyzing the cellular proliferating activity of neoplasms,it may evaluate the early differentiation tendency by which lesion is benign or malignant may be differentiated, and the prognosis of the patients may be expected accordingly.
