摘 要:目的建立结直肠癌组织微阵列,探讨上皮钙黏附素和β链接素的表达在结直肠癌发生、发展过程中的作用。方法应用微阵列制作机制作结直肠正常黏膜腺瘤腺癌微阵列,用免疫组织化学法检测组织微阵列22例结直肠腺瘤和结直肠癌63例中上皮钙黏附素和β链接素的表达。结果制作了85例患者的结直肠腺瘤腺癌组织微阵列;原发各层腺癌组织及其转移灶中出现不同程度的β链接素核阳性表达,与正常黏膜相比远处转移灶除外,β链接素细胞核内表达增加,差异有显著性意义(P<0.01);上皮钙黏附素和β链接素膜表达在结直肠癌的发生、发展过程中呈下降趋势,肌层内癌、浆膜层内癌和淋巴结转移癌灶与正常黏膜、腺瘤及黏膜、黏膜下癌相比,β链接素膜表达减少(P<0.01),差异有显著性意义;正常黏膜与各癌灶,淋巴结转移灶与原发各层癌组织之间上皮钙黏附素的膜表达差异有显著性意义(P<0.05)。结论结直肠腺瘤腺癌组织微阵列的建立使得结直肠癌相关基因或蛋白的筛选工作快捷、简便,黏附分子上皮钙黏附素和β链接素的表达水平及表达部位异常与结直肠癌的发生、浸润转移有密切的关系。Objective To develop tissue microarray of colo rectal adenoma and cancer,and investigate the role of E-cadherin a ndβ-catenin in the carcinogenesis and p rogression of colorectal cancer.Methods Tissue microarray of colorectal normal mucosa,adenoma and cancer was co nstructed by using a microarrayer.The expression of E-c adherin andβ-catenin was detected on the section s of tissue microarray by immunohistochemistry.Results Colorectal adenoma-carcinoma tissue microarray with 85patients' samples was developed.Increasedβ-catenin nuclear staining was detec ted in primary cancer tissues and their metastasis.There was significant difference between normal mucosa and cancer tissues(except for distal metastasis)(P<0.01).Membranous expression of bothβ-catenin and E-cadherin decreased d uring the carcinogenesis and progression of c olorectal cancer:β-catenin membranous staining was significantly reduced in cancer tissue in muscular and sero sal layers,lymph node and distant me tastases compared with normal mucosa(P<0.01).There were significant difference s of E-cadherin membranous staining between normal mucosa and cancer tissue in various s tages and between colorectal cancer tissue and lymph node metastasis(P<0.05).Conclusions The development of tissue microarra y of colorectal cancer and adenoma facilitates surveys of colorectal c ancer-related gene candidates.The abnormal expression level and localization ofβ-catenin and E-cadherin are co-rela ted with the carcinogenesis and progression of colorectal cancer.
